Functional Area: Benefits / Print Benefit Confirmation Form
Purpose
Use this procedure to print Choice Benefits Confirmation Forms as verification for the employee and HRSC that enrollment data has been updated in UC Flex.
Trigger
Perform this procedure when a Choice Benefit Confirmation Form needs to be printed for an employee currently enrolled in benefits OR as a batch job for multiple employees based on:
- A policy established in the HRSC that calls for benefit confirmation forms to be printed on a regular schedule, i.e. end of each day, as a batch job for employees who have been enrolled in benefits via ESS or by the HRSC staff.
- A confirmation form that needs to be printed for an employee or employees needing to make changes to their benefits due to a qualified status change, i.e., marriage, birth of a child, etc.
Employee(s) currently enrolled in Benefits.

Procedure
1. / Start the transaction using the transaction code.
Print Enrollment Form
2. / Select to the left of Other keydate.
/ Not all date fields can be selected at the same time (the period defaults to Today).

3. / As required, complete/review the following fields:

Entering a key date will display all qualified employees if no personnel number is entered.

Additional data which allows you to specify which employee groups require confirmation forms.

FULL 100% FTE
Additional data which allows you to specify which employee groups require confirmation forms.
Adjustment reason / Conditional / Reason for any benefits adjustments
Example:
HIRE
An entry is required if processing anything other than open enrollment.

/ After printing the confirmation form, you are returned to the Print Confirmation Form screen to select, if necessary, other employees for which you can print forms.

/ The system displays the message, “Spool request (number XXXX) sent to output device XXXX.”

19. / Review the displayed information.
/ A in the Status column indicates there were no errors. A in the Status column indicates there was an error and you will need to review the Descriptionin the description column.
20. / Click until you return to the SAP Easy Access screen.
21. / You have completed this transaction.

You have printed a confirmation form for an employee recently enrolled in benefits or multiple employees recently enrolled in benefits, for which verification of the enrollment is needed.
OR
You have printed a confirmation form(s) for an employee or employees needing to make changes in current enrollment elections as the result of a qualified status change.
