This facility is a residential facility for the elderly with a capacity of 4 non ambulatory residents. This facility was developed to serve consumers coming out of the state Developmental Centers and consumers who are at risk of being placed at State Developmental Centers.

Provider Statement: / Ridgewood Residential Care Home #4 is an Adult Residential Facility aimed to serve a maximum of four (4) consumers who are moving from Lanterman Developmental Center into the community. We will provide a home-like environment ensuring privacy, dignity, comfort, and security in the least restrictive manner possible.
The underlying philosophy of the home program is “normalization.” In essence, this means that we have undertaken the goal to make available to the individuals with developmental disabilities who are transitioning from Lanterman Developmental Center those patterns, conditions, situations, and opportunities and responsibilities of everyday life. We believe that each consumer deserves to experience life. It is essential that the services for each resident will be carefully planned to ensure their success in the life outside of the State Developmental Center. Each plan will be individualized for each consumer and the services and supports from the facility staff will enable each consumer be successful in their life as an integrated citizen and member of the community.
Staffing Pattern: / ADMINISTRATOR:
1. Person must 21 years of age or older.
2. Person must have an Administrator Certificate issued by the Dept. of Social Services
3. Person must have at least 2 years experience working with consumers with severe self-care challenges in an administrative or managerial capacity in a regional center vendored residential setting.
4. Person must have a current First Aid and CPR Cards.
5. Person must have an HIV/TB certificate.
6. Person must have no criminal record.
7. DSP 1 and DSP 2 Certificates required.

LEAD DIRECT SUPPORT PROFESSIONAL
1. Person must be at least 21 years of age.
2. Person must have at least 1year experience working with consumers with severe self-care challenges.
3. Person must have a current First Aid and CPR card.
4. Person must have no criminal record.
5. DSP 1 and 2 Certificate required.
6. Completion of Crisis Prevention Institute (CPI) training within 90 days from hire date.
DIRECT SUPPORT PROFESSIONAL
1. Person must be at least 18 years of age.
2. Person must have at least 12 months experience working with consumers with severe self-care challenges.
3. Person must have a current First Aid and CPR card.
4. Person must have no criminal record.
5. DSP 1 Certificate required
6. Completion of Crisis Prevention Institute (CPI) training within 90 days from hire date.
7. Completion of DSP 2 within the first year of employment.
The facility will have three direct care staff to 4 consumers during programming hours and 1 night shift staff. Administrator is on call all the time. LVN Hours will be 2 hours per day 5 days per week. BCBA is 4 hours per consumer per month. OT/PT and Dietician will be called in for initial assessments and as needed basis.
Staffing Information:
Training Requirements: / 1. All New Staff Members will undergo On-Site Orientation during the first 40 hours of employment. The following topics will be covered during the orientation:
- Client Rights
- Identification and Reporting of Abuse
- Person Centered Planning
- Medication Administration and Recording
- Emergency Plan and Disaster Action Plan
- Facility Program Design
2. The Administrator is responsible to administer the orientation and is responsible for the provision of staff training, supervision, and performance evaluation to be done on a quarterly basis.
3. The Administrator is responsible for providing on the job training to staff members to ensure that consumers’ IPP goals are implemented.
4. The Administrator is required to have 20 hours of continuing education per year in areas relating to planning and implementation of residential services for persons with developmental disabilities.
5. Direct Care Staff members are required to have 15 hours of continuing education per year in areas relating to planning and implementation of residential services for persons with developmental disabilities.
6. Direct Care staff members will be attending monthly seminars sponsored by the regional center, will receive in-service training for First Aid and CPR renewals, and other seminars that will be sponsored by the Department of Developmental Services, Regional Center and approved CEU vendors.
7. Direct Support Professional competency based training 2 will be completed on the first 6 months from the hire date.
8. The Administrator will ensure that staff members who have knowledge areas identified in the DSP training will be given proper training within one year from the receipt of notification.
All staff working with the consumers will receive monthly in-service training conducted or arranged for by the BCBA. The topics that will be discussed will be based on where staff needs extra training. More than one topic will be covered in the course of one training meeting depending on the information needed to understand the concept and depending on staff interest and relevance to the population at the Ridgewood Residential Care Home 4 program. The BCBA will conduct the majority of the in-services, but will have the option of inviting other consultants with expertise in specific areas to provide in-services in those areas.
