Region Director’s Award

Information/Instructions

2012-2013

All Regional Director’s may apply for the Florida School Nutrition Association Regional Director’s Award. One (1) strategy must be completed from each of the five (5) Initiatives. The Award is divided into three levels:

·  GOLD AWARD – Completion of 11 or more strategies. No more than one page of documentation can be submitted for each strategy. Checked strategies must be completed with documentation. (This could be 11-16 total pages when complete, not including Entry Form, Annual Chapter President’s Report, and Tracking Document)

·  SILVER AWARD – Completion of 8-10 strategies. No more than one page of documentation can be submitted for each strategy. (This will be 8-10 total pages when complete, not including Entry Form, Annual Chapter President’s Report, and Tracking Document)

·  BRONZE AWARD – Completion of 5-7 strategies. No more than one page of documentation can be submitted for each strategy. (This will be 5-7 total pages when complete, not including Entry Form, Annual Chapter President’s Report, and Tracking Document)

At the FSNA Annual Conference, the regional Director’s Gold, Silver, and Bronze awards will be presented to those Regional Directors who meet the required criteria and submit the following information:

·  A typed Regional Directors Award Entry Form by October 1st mailed/e-mailed to State President and State Office.

·  A completed Initiative for each strategy, following the guidelines below, by March 1st.

o  A one (1) inch, three (3) ring notebook, will include all documentation.

o  One (1) typed Regional Directors Award Entry Form.

o  One (1) Initiative Tracking Document for each initiative

o  One (1) typed page for each strategy (including photograph(s) and documentation).

o  One (1) page summary – Annual Regional Director’s Report. This should emphasize the major activities and outstanding accomplishments during his/her term of office.

Special Instructions:

Cover page (see General Information) and Initiatives document forms are to be included in Regional Director’s folder/binder. A page is considered 8 ½ by 11 inch white paper. A piece of documentation such as a photo must be mounted only on one side of the paper. No Booklets, CD, DVD, VHS allowed. Decorative Covers will not be included in judging. All documentation must be for the current year. DO NOT send loose or stapled papers.

Submission Dates:

·  Regional Directors Award Entry/Draft Initiatives Form - Mail/e-mail to FSNA President and FSNA Office by October 1st deadline.

·  Completed Regional Director’s Book - Mail your Regional Director’s Book (3-prong/ring folder/binder) with completed Initiatives, Annual Regional Director’s Report, and documentation to FSNA President postmarked by March 1st.
