Planning District 10 MAPP2Health’s 2012 four community health priority issues:

1. An increasing rate of obesity

2. Insufficient access to mental health and substance abuse services for segments of the population

3. Late and insufficient prenatal care and racial disparities in pregnancy outcomes

4. Tobacco use above the Healthy People 2020 goal

Re-Introducing MAPP. What’s the Game Plan? (Facilitated by Elizabeth Beasley)

-  Community driven process; HD collecting data and have vested interest in improving community health

-  MAPP strategic framework developed by National Association of City and County Health Officials (NACCHO) and Centers for Disease Control & Prevention (CDC); third time through in TJHD

-  MAPP overview schematic – includes four assessments

-  Community Health Assessment (vital records, BRFSS, local data)

-  Local Public Health System Assessment

-  Leadership Council will develop Community Health Improvement Plan

-  Previous MAPP projects; 2007-2008 and 2011-2012 (see community health priority areas above)

What does a healthy community mean to you? (Facilitated by Jackie Martin with assistance from Aaron Pannone)

·  Purpose -> Visioning will ensure the group is on the same page and we know where we’re going and what we want (i.e. our ultimate goal)

·  Completed several visioning exercises in the community and plan to do more (Rockfish Community Market and Downtown Mall, for example)

·  What does a healthy community mean to you?

-  Louisa County High School Students: Word Cloud found they focused on “community” “healthy” “everyone” “people” “clean” “safe” “recourses” “mental health” “access to food”

-  Also asked Murray, Western, Monticello and Renaissance students

-  Core Planning Group plans to go to Rockfish Community Market and Downtown Mall to get further responses; will reach out to contacts in Greene and Fluvanna as well

-  Once visioning sessions are conducted, Core Planning team will draft a few vision statements and poll the public for our Vision

Who is in the Local Public Health System? (Facilitated by Denise Bonds with assistance from Tara Blackley and Helen Dawson)

·  The 10 Essential Public Health Services Sticky Wall and String Exercise

·  Conducted the Local Public Health Assessment

-  Exercise revealed the connectivity of the organizations involved

-  “Right people in the room”; Large number of non-profits in area, though struggles continue in regards to connectivity of organizations; unspoken mission is to allow non-profits to interact to create relationships; create transparency of resources available through collaboration

Next Steps (Facilitated by Aaron Pannone)

·  Core Planning Group will follow up with visioning email

·  Will share final LPHS via e-mail and discuss at March meeting

·  Poll for meeting time and date in March

·  March meeting will determine the name of the Leadership Council and MAPP

·  Request feedback for qualitative assessment that will be developed around that time period

·  Review data for Community Health Status Assessment
