What is Patients in Control?
Patient-led commissioning - based on personalised care such as shared decision making, self-management support and joint care planning - empowers patients to manage their own treatment and care which iskey to achieving better clinical and quality outcomes, improved patient experience, better management of long term conditions, reduced emergency care and earlier intervention/prevention.
ThePutting Patients in ControlProgramme has been designed to help CCGs and Area Teams commission transformational models of personalised care, powered by the insights, knowledge and experience of patients as active partners in managing their own health and well-being.
The programme is patient-led and has been sponsored by NHS England to support CCG and Area Teams implement Transforming Participation in Health and Care[1].
Benefits of the programme
- Improving outcomes and the care experience of patients and their carers through prevention and local co-design – a vision underpinned in the Five Year Forward View:
- Delivering better value for money, maximising returns on investment, better management of chronic conditions / post-acute care and reduced demand on urgent care
- Assuring compliance with the NHS Mandate, Health & Social Care Act, NHS Constitution and NHS England reporting duties
- Supporting the successful commissioning of new approaches to self-care, for example through personal care plans, successful access to information, emotional/peer support, social prescribing and personal health budgets
- Promoting effective integrated personalised commissioning and meeting the requirements of the Better Care Fund / multi-disciplinarystrategies based on co-design with patients, carers and the voluntary and community sector.
Key components of the programme
Development support and resources for use by commissioners and other partners are being developed through two key work components:
Patients in PartnershipPatient-led materials to support commissioners / In-depth patient and carer insights conducted to identify/ confirm what is important to them to support them to manage their own condition more successfully
Resource materials developed with patients and carers to inform patient-focused commissioning
Case studies: Early Adopters and other examples of best practice
Co-design approaches to conduct work locally with patients and other key partners (local involvement, local ownership, integration)
Patient-led development of commissioner self-assessment tool / Self-assessment template designed with patients
to identify where CCG is now/what are the gaps
