Intermediate Level Critical Incident Log Book

This is a curriculum based critical incident logbook for intermediate level training (ST3 to ST4). It is expected that intermediate trainees have previously completed the core level training critical incident logbook, which covers a broad range of critical incidents. Trainees and simulation trainers should use this logbook to identify training requirements (as trainees move between hospitals).

The critical incident module within the intermediate level training syllabus (ST3 to ST4) describes 4 specific skills, which the trainee needs to show some experience of. These focus on teamwork, leadership and communication skills. It mainly requires trainees to demonstrate good leadership skills in managing incidents and their team around them. There is also a requirement for trainees to demonstrate a basic knowledge of critical incident reporting and root cause analysis. Please refer to the guidance document on completing this logbook.

Evidence of critical incident training experience also includes:

·  Resus course certificates (ALS / ATLS / APLS / MEPA / CRISP / other).

·  Presentations of critical incidents and morbidity and mortality (M&M) meetings.

·  Evidence of completing critical incident reports.

·  Producing or contributing to clinical risk guidelines.

·  Teaching on relevant courses.

Revision date: This log book should be revised in response to syllabus or training changes.

Guidance for completion

All intermediate (ST3 to ST4) trainees need to complete the critical incident module

1.  Trainees are required to complete at least one CBD for the module sign off on eporfolio.

2.  Trainees are required to show evidence of completion of the Welsh School of Anaesthesia critical incident logbook at ARCP (as per checklist)

The logbook is designed as a personal record/reflection of critical incidents and can be signed and completed by trainees :

Critical incident competency

·  Tick the experience of critical incident competency e.g. clinical, simulation, meeting, incident report or M and M or other relevant setting.

·  The competencies particularly lend themselves to major clinical events or simulation type courses where leadership is particularly important.

·  Trainee date and sign entry.

·  Tick CBD, DOPS if completed and add to eportfolio.

·  Trainee should add detail to each incident eg reflection of incident, course attended, teaching, risk meeting etc….

·  Root cause analysis competency lends itself to a teaching session focussing on this as a topic which can be arranged within departments.

The document is designed to help trainees cumulatively log their critical incident experience and particularly at this level, focussing on leadership of a team. Written reflection would be particularly useful in documenting experience.
