HDEV 470: Seminar/Practicum in Human Development

To apply for admittance to a section of HDEV 470 in SUMMER or FALL 2015 you must submit the following to the Human Development Department Office, LA3-202 no later than Wednesday, March 11, 2015 at 12:00 NOON. Applications must be submitted in person; no applications will be accepted via email or fax. Please submit:

·  The completed form below.

·  A copy of your full Academic Requirements Report located on MyCSULB. You must click “Expand All” at the top of the report (you can find this button under the “General Information” subheading and above the “Your Degree Audit” subheading). Make sure ALL Grades required for the HDEV major are visible. (Including Lower Division, Core & Foundation classes)

You must also attend ONE of the mandatory orientation meetings:

·  Monday, March 9, 2015 at 1:00 PM in PH1-112

·  Monday, March 9, 2015 at 5:00 PM in LA5-247

·  Tuesday, March 10, 2015 at 3:00 PM in LA5-247

·  Wednesday, March 11, 2015 at 8:00 AM in LA5-247
