I will not attend my IEP meeting. But I will make sure my interests, strengths, and needs are known. I will do this by:

Introduce the members attending the meeting
Ask the other members to introduce themselves
Ask someone to take notes for my IEP that I can have after
Discuss and identify your future goals after high school for the following areas:
Employment
Postsecondary training or education
Independent living
Community participation and mobility
Recreation and leisure
Describe my disability, strengths and weaknesses, and how I think I did in school this past year
Ask someone to report on my academic progress this year in relationship to where I want to be after high school and report on my strengths and weaknesses
With the IEP Team, develop my transition goals in the areas of employment/post-secondary education, independent living, and community participation that relate to my future goals
Describe my educational program for the coming school year, including goals, and develop a course of study that is meaningful to my future plans and motivates me to complete school accommodations or modifications
Identify the supports and service I need to be successful in the classroom
Design a coordinated set of strategies/activities that promotes movement from school to post-school services and identifies agencies, services, and persons that need to attend IEP Meetings or be visited. Be sure to give the school professionals permission to contact the agency or service(required under IDEA 2004)
Talk about my concerns and needs and ask the IEP members for help
