I. Agency Organization Chart
Provide an organization chart of the overall agency. If the project is a subset of the agency, highlight the position of project staff within the overall agency. If the agency is partnering with other agencies for this project, include an organization chart for the project team, illustrating the relationship between the organizations.
II. Board of Directors Roster
Provide a roster of the agency's Board of Directors, including the following information.

III. Letters of Agreement
If the applicant will be participating in a collaborative and/or subcontracting relationship, provide a Memorandum of Understanding, work agreement, or letter from the partnering organization which documents the relationship and defines tasks for which each party will be responsible. Please include the following information about any subcontractor(s):

V. Financial Report
Provide one copy only of the most recent Annual Financial Audit Report prepared by an independent certified public accountant, with balance sheets and related statements of revenues, expenses and changes in fund balances, functional expenses, and cash flow.
