Remember, Open Enrollment is your annual opportunity to make changes to your benefits elections. AllchangesmadeduringOpen Enrollment will become effective August 1, 2017. Outside of Open Enrollment, you can only make changes if you experience a family status change. See page 3 for information on making enrollment changes during the year.

Medical

Medical Benefits will continue to be administered by Aetna. However there are a few notes that need to be made.See pages 4-11 for details of the medical plans.

HIGHLIGHTS:

»For HNO / POS plan (Texas), X-ray, Lab, and Advanced Imaging In-Network coverage will be covered at 90% after deductible.

Dental

Vision

Vision Benefits will continue to be administered by Aetna. See page 13 of the details of the Vision Plan.

Life/AD&D (Basic and Voluntary), Short-Term (STD) and Long-Term Disability (LTD)

Aetna will continue to be the administrator for the Basic Life/AD&D, Voluntary Life/AD&D, Short-Term and Long-Term Disability. Inventec Distribution (North America) Corporation will continue to pay 100% of the cost for Basic Life/AD&D, Short-Term and Long-Term Disability on behalf of all eligible employees.

Membership Guidelines

WhoisEligibleforCoverage?

  • New hires are eligible for Medical and Vision insurance (Aetna) on date of hire.
  • New hires are eligible for Dental insurance (UCCI) on the 1st day of the month following date of hire.

DependentChildren

Medical: Dependents are eligible for coverage up to age 26, regardless of student status, tax dependent status, residence per marital status.

Dental: Unmarried dependents are eligible for coverage up to age 26, regardless of student status.

Vision: Dependents are eligible for coverage up to age 26, regardless of student status.

MakingEnrollmentChangesDuringtheYear

In most cases, your benefit elections remain in effect for the entireplan year (August 1st–July 31st). During each annual enrollmentperiod, you will have the opportunity to review your benefit elections and make changes for the coming plan year.

Certain coverages allow limited changes to elections during the plan year. These benefits include the medical, dental and visionplans. Under these benefits, you may only make changes to your elections during the plan year if you have a change in familystatus. Family status changes include:

»Marriage or divorce

»Gain or loss of an eligible dependent for reasons such as birth, adoption, court order, disability, death, marriage, or reaching the dependent child age limit.

»Changes in your spouse’s employment affecting benefit eligibility.

»Changes in your spouse’s benefit overage with another employer that affects benefit eligibility.

The change to your benefit coverage status must be consistent with the change in family status. For example, if you gain a newdependent due to birth, you may change your coverage status to add that dependent.

You have 30 days from the date of a family status change to turn in a completed enrollment change form to the Insurance Department. Otherwise, you must wait until the next annual enrollment period to make a change to your elections. In most cases, your election will become effective the first of the month following your request.

The goal of your medical plan is to provide competitive quality benefits for employee and covered dependents in the most cost-effective manner. The employee’s premium contributions for health insurance cover only a small portion of the full cost. The remainder of the cost is paid by Inventec Distribution (North America) Corporation.

POS (Point of Service) Plan – Texas Employees

The POS plan provides two levels of benefits, In-Network and Out-of-Network coverage. The POS plan utilizes the Aetna Choice Network.

The In-Network Calendar year deductible is $500 per individual/$1,000 per family.

In-Network Out-of-pocket maximum is $3,000 per individual/$6,000 per family.

Primary care physicians office visit copay is $35 and a Specialist copay is $50.

PPO (Preferred Provider Option) Plan – California Employees

The PPO plan provides two levels of benefits, In-Network and Out-of-Network coverage. The PPO plan utilizes the Open Choice Network.

The In-Network Calendar year deductible is $500 per individual/$1,000 per family.

In-Network Out-of-pocket maximum is $3,000 per individual/$6,000 per family.

Primary care physician office visit copay is $35 and a Specialist copay is $50.

In-Network Coinsurance coverage is 90% AFTER Deductible.

H.S.A (Health Savings Account)PPOPlan

The H.S.A (Health Savings Account) PPO plan also provides two levels of benefits, In-Network and Out-of-Network coverage. This plan utilizes the Open Choice Network.

The In-Network Calendar year deductible is $2,000 per individual/$4,000 per family.

In-Network Out-of-pocket maximum is $3,000 per individual/$6,000 per family.

In-Network Coinsurance coverage is 100% AFTER Deductible.

By selecting an H.S.A PPO plan, you qualify to contribute tax-free money into a health savings account (HSA). Your HSA funds then earn tax-free interest, and can be used tax-free to pay for qualified medical expenses (preventive care items are always covered 100%). HSAs are similar to retirement accounts in that they rollover year-to-year, they remain yours if you change jobs or retire, and the balance can be invested per bank options as well.

With a H.S.A PPO health plan, instead of makingcopayments at the time of service, you will have a deductible to meet. A deductible is the amount you are required to pay out-of-pocket for services before your health plan benefits kick in. While the deductible may be higher deductible than you’re used to, you will have tax advantages for this plan. For example: reduced federal income taxes, tax free interest on dollar gains within the H.S.A account/mutual fund investments, and tax free withdrawals for qualified medical expenses.

Please NOTE that to qualify to enroll on this plan, the following requirements apply:

1)You must not be enrolled on another health plan (ie. spouse’s PPO plan).

2)You must not be enrolled in Medicare.

3)You cannot be claimed as a dependent on someone else’s tax return.

HMO(HealthMaintenanceOrganization)Plan

The HMO plan is the gatekeeper concept. This means you will need to designate a Primary Care Physician within the HMO Network, who will recommend specialized providers as needed for your healthcare. You are not required to have preauthorization for obstetrical or gynecological care also pediatricians can be a primary care physician for children. The HMO plan utilizes the HMO Network.

Out-of-pocket maximum is $3,000 per individual/$6,000 per family.

Primary care physician office visit copay is $20 and a Specialist copay is $45.

Dental—PassivePPO

Under the United Concordia dental plan, you may see any dentist that you choose. However, in-network dentists have agreed to accept reduced fees for the services they provide. They have also agreed not to charge you any amount that exceeds the allowable amount, aside from deductibles, coinsurance and services that are limited or not covered under the Plan. This will reduce your out-of-pocket expenses.
