Please provide the following information so that I can prepare the best recommendation possible. Although I may know much of the information, this will make sure I do not miss something and that I have everything together when I sit down to prepare your recommendation. Which is the most rapid way to contact you?
For what would you like me to write you a recommendation (e.g. scholarship, academic admissions, employment)? Provide any pertinent information about the position/school/etc. that you can. Are there specific qualities that they are looking for so that I can highlight those in the recommendation?
List the following:
If applicable: To whom should I address the letter?
Address to send the letter OR how it will be submitted (e.g. Interfolio, AACOMAS, etc.)
What is the due date or deadline?
List course(s) of mine, including labs, you have been in and when you took the course (semester & year).
Why are you interested in the school or activity that the recommendation is for? If the recommendation is for a school, why are you interested in that field as a career?
Have I filled out a recommendation for you previously? If so, when and for what was the recommendation?

B. The questions in this section are useful, but some may not apply to your situation.
List events (e.g. attended CANCAS Fall Workshop), groups (e.g. Walker Biology Club), or activities (e.g. prospective student visitation) we have been involved in together.
List employment or other pertinent activities you have been involved in related to the biology department.
List honors or awards, extracurricular activities, volunteer/community service activities, work experience.
Highlight relevant activities: e.g. if you are interested in pharmacy school, things like pre-pharmacy club and work experience in a pharmacy.
List internships, co-ops or other pertinent experience.
What is your overall and science GPA? If applicable, what did you get on any pertinent standardized tests (e.g. MCAT, PDAT, PCAT, etc.). If your GPA or test scores are not as high as you would like, briefly explain any obstacles (or other factors) to achieving a higher GPA or test score?

C. Attach (or send separately) a current copy of your transcript.
Please provide me with a current copy of your transcript that you can obtain for free through Web Access. The easiest way to do this is to “print” the transcript page as a pdf. If you do not already have a tool, PDF Creator is a free software that you can download from the internet. Since most recommendations require the evaluator to address a student’s academic performance, this will allow me to give a more accurate representation of your performance.

Many students I do not see again once they leave for their future plans so I would like to have a way to remember you. If you do not have a convenient photo or it is too old, please stop by and let me take your picture.
