Letter of Recommendation Request Form

Most college applications require a Letter of Recommendation from your school counselor. I am planning to write a bulk of letters for rising seniors this summer since many of you will be applying in the fall semester and the beginning of the school year tends to be a busy time. Letters will be completed on a “first come, first serve” basis with priority given to students who submit a Student Credentials Sheet during the summer months.

To help me write the best and most informative recommendation to accompany your college application, answer the questions below. You must return this form at least THREE WEEKS before the application deadline so please plan accordingly.

Type your responses and write in complete sentences. The Activity Record must also be completed or submit a copy of your resume.

  1. List the colleges you need recommendations for. Why do you want to attend that particular school and why you see yourself “fitting” in at that school. Have you visited the school?
  1. What is your career goal and what educational steps do you plan to take to reach it? Do you have a major at this point? If so, what is it?
  1. What has been your favorite class in high school? Why?
  1. What words would your teachers use to describe you?
  1. List at least three adjectives you would use to describe yourself.
  1. What would you say are your strengths or best talents?
  1. List some areas you would like to strengthen. Explain.
  1. If you were to start high school again, are there ways in which you would function differently? Identify these areas and explain.
  1. What would you like someone to say about you in a letter of recommendation?
  1. Have there been any circumstances in high school, personal or family, that have affected your academic performance either positively or negatively? Please explain.
  1. Is there anything else that you want the college admissions office to know about you that is not mentioned anywhere else in your application, essay, portfolio, and other recommendations?
