Field Trip Permission Form

Dear Parent or Guardian,

Your son or daughter has been invited to participate in a team field trip to the Carnegie Science Center on Friday, September 30, 2016. We will be leaving at 9 a.m. and returning by 2:00 p.m. The students will be able to explore the exhibits, the submarine, and the Sportsworks areas of the science center. All of these activities refer to topics covered in their science class. Students will also be attending the Omnimax presentation called “National Parks Adventure” to familiarize them with the beauty and wonder of our park system during its centennial year. The cost of the field trip and the bus is $16.00.

Students are required to bring their own lunch in a paper bag with NO glass bottles. They are not permitted to bring backpacks or large bags with them. Permission slips and money are due by Monday, September 26, and are to be given to their homeroom teachers. Checks must be made out to SVMS.

If your son or daughter has any health issues that may be of concern, please fill out the health form on the back of this permission slip.

·  Please throw gum away when you arrive.

·  Eat only in designated areas.

·  Do not drop items over the railings.

·  Please walk. Do not run.

·  Elevators are only for people who need them. Please use the ramps and stay to the right.

·  You must stay in line to keep your turn at the exhibits. We do not permit place holding or line cutting. Anyone caught cutting in line will be sent to the back of the line.

·  Keep your group ID tags on and visible at all time.

·  Leave personal items such as coats, hats, and bags on your bus or in a locked locker. Lockers are behind the ticket counter on the first floor. They are first-come, first-served, and cost 50 cents.
