B.  PROGRAM OVERVIEW

Woodville Alliance is a not-for-profit community organisation providing disability services, early childhood education and childcare and community services including: mental health prevention, early intervention, family and community services and programs for children and young people.
We work mainly in South Western Sydney and Western Sydney with centres in East Fairfield, Carramar and Auburn. Strong Minds is one of the programs in the Community Initiatives Team which is funded by the Department of Social Services (DSS) under the Family Mental Health Support Service (FMHSS) program across three locations in Western and South Western Sydney. This program offers early intervention supports to vulnerable families of 0-18 year olds who are at risk of or affected by mental illness to reach their potential.
The key aims are to ensure:
-  Children and young people have improved emotional health and well-being.
-  Children and young people are able to better manage the different aspects of their lives.
-  Families and carers are better able to get support to help their children and young people.
-  Communities have a better understanding of, and response to, mental health issues that affect children and young people.
The primary objective is to develop early intervention support to vulnerable families of children and youth at risk of or affected by mental illness to reach their potential. This will involve a focus upon three areas of service: Intensive Long-term support, Short-Term Support and Community education and outreach.

C.  JOB PURPOSE

The Team Leader is responsible for the provision of strong leadership within the Strong Minds team. The role is pivotal in the supervision and support of team members to ensure high quality service provision to clients and in compliance with our funding requirements.
The Team Leader will provide leadership and support change management in the continued development of our service delivery model in line with best practice family mental health support services.
This position also provides direct case management services to clients.

D.  RELATIONSHIPS

In order to ensure best practice and quality service delivery is provided within Woodville Alliance (WA), the Community Initiatives Team (CIT) and the Strong Minds team, this position must develop and maintain internal and external networks and relationships. These may include:

·  Work closely with the Strong Minds Manager and the General Manager, Community Initiatives to ensure the program is effectively delivered and continuously improved

·  Internally work with others within WA and CIT to promote collaboration and ensure the delivery of quality services to clients

·  Work with families, children and young people and the other services who may be supporting them to ensure a co-ordinated client centred approach to case planning and achievement of client goals

·  Connecting with other workers focused upon early intervention and mental health programs from other service providers (government & non-government);

·  Liaising and collaborating with other organisations such as local schools; local councils, Centrelink, child protection agencies and housing providers.

·  Participation in relevant local community events to promote the Strong Minds program and to connect with residents

·  Participation in sector forums such as interagency.

Key Result Area 2 / Ensures that services delivered are in line with the Family Mental Health Support Service Program and other funding and policies and procedures.
KEY TASKS
1.  Lead, support and monitor the team to ensure effective client-centered and trauma-informed practice
2.  Lead, support and monitor the team to ensure delivery of support services in line with policies, procedures and funding requirements
3.  Establish, implement and monitor effective referral partnerships and procedures
4.  Establish, implement and monitor effective client assessment procedures
5.  Establish, implement and monitor collaboration with other agencies to ensure effective holistic services to clients
6.  Promote safety of workers and clients and ensure effective communication, management and reporting of incidents and risks
7.  Undertake case work based on a caseload assigned to you as agreed with the Manager, Strong Minds
Key Performance Indicators (KPIs) for the tasks
·  The split of intensive and short term clients is consistent with funding requirements (Target: 100% of short term and intensive cases allocated to the team achieved)
·  Positive feedback from referral partners (Target: >90% of referring organisations report they are satisfied with the SM service)
·  Clients satisfied with the service they receive from the Strong Minds Team (Target: >85% of clients)
·  Clients are satisfied with the level of decision making and control they have over the service they receive from Strong Minds team (Target: >80% of clients)
·  Review of client outcomes indicates clients are meeting their case plan goals (Target: >80% of clients meet their case plan goals)
·  All clients classified as requiring intensive support, must have a family action plan (in partnership with the family) within 30 days of the initial assessment (Target: 100% of intensive clients with a case management plan)
·  Client records are accurate and timely (Target: >90% of case notes are completed within 24 hours of the service contact/ session)
·  Case workers utilize the Guardian App and other procedures provided by Woodville Alliance to promote worker safety (Target: all case workers following safety procedures 100% of the time)
Key Result Area 3 / Contribute to continuous improvement.
KEY TASKS
·  Facilitate and participate in the evaluation, review and audits of the Strong Minds program
·  Facilitates delegated continuous quality improvement activities ensure approved recommendations and corrective actions within your area of responsibility are implemented.
·  Contribute to the development, continuous improvement and evaluation of the Strong Minds program and other related programs.
·  Keep up to date on the latest research and best practice approaches to family support, youth, and mental health strategies.
·  Report on achievement of KPIs, risks and issues as required by Woodville Alliance and external funders and partners.
·  Participate in regular clinical supervision around specific cases as well as general supervision based upon KPIs in service planning and provision to overcome challenging issues or relationship breakdowns.
·  Prevent, communicate and report risks and incidents in line with WA policy and procedures.

·  WA internal and external audits find your team is compliant with policies and procedures (Target: 95% positive outcomes on all service delivery audit measures)
·  Client satisfaction surveys are completed (Target: >95% of clients complete survey)
·  Complaints from clients, other agencies or team members are addressed effectively and promptly (Target >90% of complaints addressed in less than 1 month)

G.  RECRUITMENT CRITERIA

The essential knowledge, experience, skills and personal attributes required for the position
1.  Tertiary level qualifications in the areas of Social Work, Psychology or equivalent
2.  Demonstrated experience in case management and therapeutic interventions at a minimum of 3 years and knowledge of case management principles and best practice
3.  Experience in working with families, children and/or young people with or at risk of mental illness and demonstrated knowledge of mental health issues
4.  Demonstrated experience in leading a small team of employees and improving team performance
5.  Demonstrated understanding of working with culturally and linguistically diverse clients, especially those from refugee backgrounds
6.  Effective written and verbal communication skills
7.  Fluency in the use of client information systems and computer applications including Microsoft Word, Excel and Powerpoint
8.  Commitment to social justice, safe work practice, quality assurance and non-discriminatory practice.
9.  A current driver’s licence recognised by the NSW RTA
