Course Model: This is an “outcome-based” course. The student will need to pass each chapter/unit by at least 80% accuracy as measured on assignments and assessments . The learning targets for assignment are outlined below. The student has choice in the learning methods to reach understanding of the materials. The daily work is completed off-campus or during advisories. During your on-campus time, you will have a weekly one-on-one meeting with your teacher/advisor to support your learning. All curriculum and instructional materials to complete the course are available at school for check out or to use on campus. Students complete a monthly self-evaluation to reflect on learning and establish monthly goals and outcomes.

To build understanding of each learning target, the student will:

1.Keep a portfolio of all completed work

2.Come prepared with weekly work in your binder at your advisory meeting.

3.Design any projects and resources that can increase understanding and integrate with other subjects. (This is an optional activity to increase understanding and provide connections to other subjects.)

4.Complete all assignments and assessments listed at 80% or better level

5.Other: UPDATE YOUR WEEKLY HOURS AND ACTIVITIES BEFORE WEEKLY MEETING WITH ADVISOR

At your monthly teacher evaluation meetings, the student will:

7.Review the learning targets for the next month and update learning plan

8.Determine if satisfactory progress has been made on your learning plan based on 80% scores on all assignments.

Student Weekly Log and Self Evaluations Fall Semester 2016-17

This document is the running log of student goals and activities connected with your independent contract at Eagle Harbor High School. This document should be “SHARED” with editing rights to your advisor and to at least one of your parents. This is also a place where your advisor may make notes of agreements, deadlines, and other comments.

WEEKLY…the student is expected to update this document PRIOR TO YOUR WEEKLY ADVISOR MEETING.

MONTHLY…the student is expected to total hours, write a short reflection for the month and use this document to update your WSLP FORM...that document is your VERY IMPORTANT contract document.

END OF TERM...the student is expected to complete a final reflection found at the end of this document.

PARENTS...This document will give you a more detailed understanding (than skyward can provide) of the student’s contract studies. You are asked review this document at least monthly.
