DISCLOSURE CHECKLIST

In order to complete this case, we will need to provide certain information to the other attorney/party. I need you to provide me as soon as possible the information that I have marked on this checklist.

Documents for Divorce Proceeding:

DETAILED SUMMARY: Compose a detailed summary of case and facts that should be brought to my attention along with timeline of events.

INCOME TAX RECORDS: Personal, corporate, partnership, joint ventures, or income tax return (state and federal), including all attachments, W2, 1099, and K1 forms in your possession and control for the years ______.

PERSONAL PROPERTY TAX RECORDS: Provide your personal property tax returns (filed in this state or elsewhere at any time throughout the marriage.)

OTHER TAX RECORDS: Other tax returns for any other returns you signed for the years ______.

o  PERSONAL INCOME: Pay stubs for the preceding three months.

FINANCIAL AFFIDAVIT: A completed Financial Declaration. (Please complete the Financial Declaration provided with this document.)

o  REAL PROPERTY: All deed, closing statements, tax bills, appraisals, mortgages, security agreements, leases, and other evidence (including monthly payments and present principal and interest balance) of any type of interest or ownership, whether as owner, co-owner, fiduciary, trust beneficiary, manager, or otherwise, during the term of the marriage; together with evidence of all contributions in case. Provide a statement showing the presenter otherwise, made by you on your behalf toward the application of such real estate during the marriage or afterward.

PROPERTY DESCRIPTION: Provide a deed showing the property description on any property you own (not a tax notice).

PERSONAL PROPERTY: Prepare a personal property list of all personal property that you own, either individually, jointly, as trustee or guardian, or through any other person or entity during the term of the marriage. Include a description of the asset, the approximate current value of the asset and the person who should be awarded the asset.

BANK INFORMATION: Provide all monthly bank statements, check stubs or registers, deposit slips, canceled checks, and bank charge notices on personal and business accounts and certificates of deposit for all bank accounts for the last year from each and every bank account, whether personal or business, whether checking, saving or money market.

STOCKS BONDS AND MUTUAL FUNDS: Provide certificates held individually, jointly or as a trustee or guardian, of any and all stock, and/or stock options owned during the marriage (can obtain current value of the stock on the internet at www. Wallstreetcity.com

o  OTHER ASSETS: Provide documents showing any other asset not previously disclosed (i.e. frequent flyer miles, etc.)

o  VEHICLES: Provide the make, model, VIN number, and current value of all motor vehicles owned by you individually or jointly, including airplanes, automobiles, boats or any other type of motor vehicle (you can obtain the value information from internet sites suck as www. Kbb.com or

SEPARATE PROPERTY: Provide any and all documentation showing that you received money from other sources to purchase property during the marriage or other proof that shows any portion of the martial property is your separate property (i.e. gifts from parents, inheritance, pre-martial property, etc.)

BUSINESS INTERESTS: Provide all business employment partnership agreements indicating any kind of personal interest to which you may have been a party during this marriage.

o  BUSINESS ASSETS: List all assets of your business, including the approximate current value.

ACCOUNTS PAYABLE AND RECEIVABLE: Provide ledgers in your possession and control that are personal and business related, together with all accounts and journals.

o  DEBTS: Provide current credit card statements and monthly statements for any and all other debt for which you have a present balance or for which you claim your spouse should assist in paying (i.e. student loan, car payments, credit cards, etc.)

o  LIFE INSURANCE: Provide certificates of life insurance currently insuring your life or the life of any other person in which you are a primary or contingent beneficiary, showing face value of the policy and any cash value in said policy.

o  RETIREMENT: Provide current statement of any and all retirement accounts, pensions, profit sharing funds, deferred compensation funds or other kind of plan owned by you or any corporation in which you are or have been a participant during the marriage.

OTHER INFORMATION: Any documentation you believe is relevant.

o  FAULT: Provide a summary of fault during the marriage.

CUSTODY/VISITATION: Provide a summary of concern regarding custody and /or visitation.
