“Connecting Children and Families to Supports and Services,” will provide a framework to conceptualize the extensive system of supports and services that link families to needed services.
Keep in mind that the agencies you are introduced to reflect SOME but not all resources. Learning about all of the available resources for families will be an ongoing process for you.

Activity #5 In this activity, your are required to check out specific programs and information (see the links below). In addition, you must link to the HHSC publication that explains community-based programs to families. While you do not need to remember all of the community-based programs, you should know where to find the information so that you can share it with families.

HHSC has developed a guide you can use in talking with families about home and community-based services that may be available to them through Texas Medicaid. This guide, “Home and Community-based Services: Which Programs Do What?” provides information about six programs that provide home and community-based services. These programs are sometimes called “Medicaid waivers” or “waiver services.”

Review the section that covers services for children with disabilities. Some of your families will need to utilize these services in the future. Some require signing up early so the service will be available when the family needs to access it. Think about how you will use this resource. Do you know when to talk to families? Do you have questions about how to talk with families about this topic? Talk to your supervisor.
