1. One page resume
2. A narrative statement no more than two pages in length discussing your academic achievements, involvement on campus and in your department and your commitment to the field as it relates to the award.

Upon review of applications, award recipients will be notified by March 8, 2013. Your attendance at the Awards Program and Reception on Thursday, May 2 is mandatory should you be chosen as an award recipient. Return your application, preferably by e-mail, to Maureen Gilbert . Dianne Bartz Berthold Scholarship
The Dianne Bartz Berthold Scholarship provides monetary support for an undergraduate student with a physical disability who uses DRES services and demonstrates financial need. Preference shall be given to veterans of the US military.
Pamela Borelli & Family Leadership Achievement Award
The Pamela Borelli & Family Leadership Achievement Award is presented annually to two students with disabilities who have maintained a C+ or better GPA while demonstrating leadership in a student organization, with preference toward but not limited to Greek campus social organizations and well-balanced involvement in academics and other campus activities. This is a monetary award.
Glenn Hebert Scholarships (2)
Created by University of Illinois alumnus, Glenn Hebert (MS ’78), these scholarships
provide monetary support for students with a disability registered with DRES at the University of Illinois at Urbana-Champaign with a preference for students with Cerebral Palsy, Learning, Cognitive, or Psychological Disabilities. These scholarships may be renewable.
Carol Ann Kostka Award
The Carol Ann Kostka Award was established in memory of Carol Kostka. Ms. Kostka attended the University of Illinois at Urbana-Champaign from 1980 through 1987. She was studying to receive a master’s degree in English. This is a monetary award.
The award is given annually to enhance educational opportunities for students with disabilities who utilize the programs and services of the Division of Disability Resources and Educational Services (DRES). Recipients of the award must demonstrate academic excellence.
Joseph R. Larsen Memorial Scholarship
This annual scholarship provides financial assistance and merit-based support for students with disabilities who are served by DRES. It was created in memory of Dr. Joseph R. Larsen who served as the director of the Division of Disability Resources and Educational Services from 1985 until his death in 1989. Dr. Larsen was extremely active in pursuing programs and services for people with disabilities. In 1987, in recognition of his efforts, he was inducted into the National Hall of Fame for Persons with Disabilities.
Jeffrey I. and Lisa Livovich Award
Created by University of Illinois alumnus Jeffrey Livovich (BS ’77, MD ’81) and his wife, Lisa, this award provides monetary support for students with disabilities at the University of Illinois at Urbana-Champaign who are served by DRES.
Timothy J. Nugent Award
In 1970, a special award was established by members of Delta Sigma Omicron, Alpha Chapter, to honor Professor Timothy J. Nugent, founder and director emeritus of the Division of Disability Resources and Educational Services. The Timothy J. Nugent Award is presented annually in recognition of service performed by a student to the rehabilitation program and to his or her fellow students. The recipient is judged on the following attributes: personal integrity, human dignity, intellectual endeavor, pride in self, and concern for others. A portion of the funding will be used to purchase services and/or equipment for benefit of other University of Illinois students with disabilities.
James E. Seybold Scholarship
Created by James Seybold’s mother, the late Charlotte Reid Simon, this award provides financial support to students with disabilities who are pursuing degrees in the College of Media.
Herbert G. Schaefer Scholarship
The purpose of the Herbert G Schaefer Scholarship is to provide scholarships to students who have a hearing impairment and to support assistive communication and information technologies used by students with hearing impairments.
Bill Stewart Memorial Award
The Bill Stewart Memorial Endowment Fund was established by the Moore Foundation of Indianapolis, Indiana, to foster student leadership in the Division of Disability Resources and Educational Services (DRES) at the University of Illinois at Urbana-Champaign. This is a monetary award.
The primary focus of the Fund is to support an annual award to an undergraduate or graduate student with a disability who demonstrates spirit, courage, and concern for others. The unique aspect of this Fund is that the award recipient will “Pass the Baton” by choosing the manner in which a portion of the income from the Fund will be used to purchase services and/or equipment for benefit of other University of Illinois students with disabilities. This gives one student the opportunity to give to another and to help improve the quality of the life for fellow students with disabilities.
Donald W. Swift Scholarship
Funded by the family of Donald Swift (1924-1993), who was the first student using DRES services to graduate from the University of Illinois, this annual scholarship provides financial assistance and merit-based support for students with disabilities who are served by DRES.
