Service description / On Demand Availability of Palliative Care Drugs
This service is aimed at the supply of specialist medicines, the demand for which may be urgent and/or unpredictable. The pharmacy contractor will stock a locally agreed range of specialist medicines for palliative care and will make a commitment to ensure that users of this service have prompt access to these medicines at all times agreed with the PCT.

Aim of service

To improve access for healthcare professionals, carers and patients to these specialist palliative medicines when they are required, by ensuring prompt access and continuity of supply.

Description of service to be provided

1.  The pharmacy holds the specified list of medicines required to deliver this service at all times and will dispense these in response to NHS prescriptions presented. The pharmacy may also hold specialist equipment, linked to the medicines on the list, for example syringe drivers and associated equipment.

2.  The pharmacy contractor has a duty to ensure that pharmacists and staff involved in the provision of the service have relevant knowledge and are appropriately trained in the operation of the service.

3.  The pharmacy should maintain appropriate records to ensure effective ongoing service delivery and audit. The pharmacist must make the PCT aware if they are unable to provide the service, especially over bank holidays.

How the service will be funded

The PCT will reimburse the cost of any medicines in the formulary once it exceeds its expiry date.

Audit

The pharmacy will comply with any post-payment verification of payment claim forms undertaken by the PCT.

Performance monitoring of this service specification may be undertaken at the time of the pharmacy contract assessment visit.

Monitoring

The PCT reserves the right to request evidence or information that the contractor is providing the service in a way that is safe, convenient and in accord with the requirements of this specification. The contractor is required to comply with all reasonable requests for evidence or information.

Notice

This enhanced service agreement may be terminated by either party by the giving of one months notice in writing.

Please send the reimbursement form to the Primary Care Commissioning Team for the attention of Wendy Illsley. Please ensure that the packaging of the expired medicine is attached to the reimbursement form. Without this evidence, payment will not be made.

·  *Morphine Injection 30mg/ml
·  Diamorphine Injection 100mg
·  Oxycodone Injection 10mg/ml
·  Haloperidol Injection 5mg/1ml
·  Hyoscine Butylbromide 20mg/ml
·  Cyclizine Injection 50mg/ml
·  Levomepromazine Injection 25mg/1ml
·  Midazolam 5mg/1ml – 2ml amps
·  Water for injections 10ml
·  Dexamethasone Injection 4mg/ml
·  Diazepam Rectal Tubes 10mg (x 2)
·  Metoclopramide 5mg/ml – 2ml amps
·  Morphine Sulphate Oral Solution 10mg/5ml (2 x 100ml)
Please note, morphine injection is first line choice unless volume of injection necessitates the switch to diamorphine.
Diamorphine 10mg injection has been removed. If these cannot be used to supply against a prescription you will still be able to claim for the cost should they expire. This applies to pharmacies who signed the 08/09 SLA.

Nurse independent prescriber prescriptions are lilac and will have the nurse’s name, NMC registration number, designated PCT or district, Practice code (K code), the nurse’s contact number and a FP10P number. For verification of any other nurse, not included below, please contact Gail.
