Working together to prevent heart attacks, strokes, and diabetes complications
University of Best Practices Goals: Heart attack, stroke,and diabetes-complications prevention focused on patients diagnosed with hypertension, heart disease and/ or diabetes through lipid, blood pressure, and blood sugar management.Achieve National HEDIS 90th Percentile (“A Grade”) Targets:
75% of hypertensivepatients with blood pressure controlled: <140/90 mm Hg
73% of patients with cardiovascularconditions with lipids controlled: LDL-C < 100 mg/dL
71% of diabeticpatients with blood sugar controlled: HbA1c <8
59% of diabeticpatients with lipids controlled: LDL-C < 100 mg/dL
Learning objectives:
- Learn how one large Sacramento medical group achieved the top 10% nationally in the control of blood sugar for diabetes.
- Learn effective diabetes care implementation strategies for educing physician work load such as team based care and leveraging EPIC E.H.R
- Learn practical strategies for using the ambulatory insulin guidelines for type 2 diabetes patients
1.Learn how to establish rapport and therapeutic partnerships with Veterans
2.Learn about the link between Agent Orange exposure and its association with adult onset diabetes mellitus and ischemic heart disease
