PURPOSE OF JOB TO LEAD ON THE DEVELOPMENT AND IMPLEMENTATION OF OPERATIONAL SYSTEMS THAT EFFECTIVELY SUPPORT YOUNG PEOPLE WITH LDD AGED 16+ AND TO INFORM STRATEGIC DEVELOPMENT IN THIS AREA

AND

RESPONSIBILITIES

1.  To manage the Transitions Team in respect of those young people aged 16+ who will require support as they move into AdultsServices.

2.  To ensure that the appropriate assessments required by Children’s and Adult Services have been carried out and recorded in accordance with the relevant service requirements.

3.  To identify, in conjunction with other parts of the LDD Service, at the earliest possible date, those young people who are likely to require support from Adults Services.

4.  To work in partnership with Adults Services to develop appropriate flexible service provision to meet the needs of young people.

5.  To assist and support other parts of the LDD Service to help parents of all children with learning difficulties and disabilities understand the range of options that are available to young adults.

6.  To contribute to transition planning at key phases and to support the development of a coherent Transitions Strategy.

7.  To develop clear Transitions Pathways into Adults Services and other Post 16 provision/services.

8.  To work collaboratively with other professionals across health, education, social care and other partners to develop a person centred approach to transition.

9.  To develop an appropriate key worker system for all young people with LDD.

10.  To work collaboratively with partners, including colleges. LSC and universal providers across the city to enable them to understand and consider the importance of effective transitions.

11.  To support the overall matrix management system across the wider LDD Services, as required.

12.  To identify gaps in Post 16 provision for young people with LDD and to work with partner agencies on addressing these gaps.

13.  To prepare and present reports to Senior Management, as required.

14.  To manage the performance (via supervision and appraisal) of staff within the Team.

15.  To undertake any other duties and responsibilities appropriate to the grade and purpose of the job as may be determined between the post holder, management and the appropriate trade union.

16.  The post holder must, at all times carry out their duties and responsibilities in accordance with Sheffield City Council policies and procedures, in particular those relating to Equal Opportunities and Health and Safety.

CONDITIONS OF SERVICE

The conditions of service applicable to the post are those laid down by the appropriate bodies as applied by the Sheffield City Council.

The post holder is reminded that the nature of the work entails attendance outside the normal office hours and acceptance of the terms and conditions of employment indicate a willingness to undertake such duties.

Whilst the appointment is made to manage a special area of service the post holder will be expected to cover the absence of, or support, other Team Leaders as may be required by the Service Manager.

Skills/Knowledge

Ability to manage social care staff within the context of children and adults social care legislation.
Ability to form partnerships with young people and carers in challenging circumstances.
Ability to work as part of a team.
Ability to manage staff.
Knowledge of relevant education & social work legislation in relation to LDD.

Excellent written, verbal and presentation skills.

The ability to think laterally and to problem solve within legislative policy and constraints.

Experience/Qualifications/Training

Social Work Qualification (SW Degree, DipSW, CQSW, CSS or equivalent) / Application form/copy of certificates
Significant experience of working with families of children with disabilities or with adults with learning difficulties or disabilities.
Complex assessment and case planning within either children or adult disabilities service.
Understanding of funding streams and budget management within children and/or adult services.
Significant multi-professional working experience.
Experience of working in a large organisation.
