REQUIRED DOCUMENATION FOR FINANCIAL AID APPLICATIONS
REMINDER: CLOSING DATE FOR FIRST YEAR STUDENTS FOR 2016 IS FRIDAY, 13 NOVEMBER 2015 @ 15H45 PM
  • Certified ID copy (applicant).
  • Certified copy of matric results (1st year students).
  • Certified copies of parents or guardians ID and siblings (or birth certificate for siblings).
  • Proof of income for parent/guardian:- payslip; grant slip (old age, disability or child);
  • If parent is receiving weekly/bi-weekly wages/salary. Please provide a payslip for each week of a specific month.
  • Affidavit stating amount pm/pa in case of employment where there is no proof of income & bank statement. Parent must state what their job is in the affidavit.
  • Parent/guardian unemployed – affidavit with amount received to sustain the family. MUST PROVIDE: UIF card/letter from the Department Of Labour stating parent is unemployed or report from Social worker or religious leader (must have signature and organizational stamp on the letter).
  • Maintenance letter from the court (if parent is receiving maintenance).
  • If parent owns a company – provide certified audited copy of recent balance sheet/financial statements AND 3 months bank statements for the company and both parents.
  • If parent divorced, copy of divorce order.
  • If parent deceased, copy of a death certificate and marriage certificate. MUST PROVIDE: Letter from the Department of Home Affairs confirming single parent status if parent was not married to deceased parent or report from Social worker or religious leader (must have signature and organizational stamp on the letter)
  • If parent single, affidavit from a parent confirming single status and whereabouts of other parent and amount that other parent contributes. MUST PROVIDE: Letter from the Department of Home Affairs confirming single parent status or report from Social worker or religious leader (must have signature and organizational stamp on the letter)
  • If leaving with grandparent/ legal guardian, affidavit should accompany the documents to confirm the whereabouts of biological parents. MUST PROVIDE: Report from Social worker or religious leader (must have signature and organizational stamp on the letter)
  • Siblings older than 21 must provide proof of income or if unemployed, and affidavit confirming unemployment.
  • Student is self-supportive. Affidavit with amount received to sustainthemselves. MUST PROVIDE: A report from Social worker or religious leader (must have signature and organizational stamp on the letter).
  • Proof of registration if more than one student at university (tertiary level).
IMPORTANT NOTE:
ONLY forms with ALL supporting documents will be accepted.
Faxed and scanned documents will NOT BE ACCEPTED.
All documents must be CERTIFIED ORIGINAL (No copies of certified documents will be accepted)

Avoid long queues and hand in your application before 13 November 2015, If you have outstanding documents you then still have time to collect these documents and hand it in before the closing date – without wasting your time in long queue!!!!!!!!!!!
