Explanation of Forms for 2017-2018
Archdiocese of Newark CYO Leagues
TEAM FORM (this form gets returned)
There are two statements that coaches must sign off on that they have done these two things.
You will also see that at the bottom of the page it requires FOUR signatures. That of the Athletic Director, Head Coach, the Pastor of the Parish and also the Principal of the School. If your team has ONLY children from the parish and no children from the school, or if the Parish does not have a school - you do not need a Principals Signature. The Head Coach and Pastor’s signatures are required regardless.
TEAM ROSTER FORM (this form gets returned)
You will notice we ask you to specify if the child is from the Parish School or from the Parish Religious Education Program. As per CYO Rules if the child DOES NOT meet either of these criteria they CANNOT PLAY IN CYO SPORTS. You will also notice at the bottom of the Roster Form we are asking for FOUR Signatures. If you have no children that play from the Parish School or from the Religious Education Program you do not need to get those signatures. The Head Coach and Pastor’s signatures are required regardless. If you have children from the Parish School and Religious Education Program you need all four signatures. This form has been placed on the website as a PFD and fillable PDF – but must still be printed out for signatures.
Coaches Code of Conduct Form (this form gets returned)
This form is to be signed by every coach from your program and returned with the rosters and team forms. It reviews basic expectations set forth by the Office of Youth & Young Adult Ministry for all coaches
PLAYER PERMISSON FORM – this medical release is to be filled out and KEPT BY THE COACH in case of an emergency. These DO NOT get returned to us.
CYO CODE OF CONDUCT FORM – this form is to be gone over, signed and KEPT BY THE
COACH. These DO NOT get returned to us.
