1) CLEC Company – The name and address of the CLEC Service Provider.
2) Contact Information – CLEC contact information based on the business function supported by Sprint. This includes information for the service order, repair, billing, security, LNP and 911 record management functions. For each function, please include the following information:
  • Name of CLEC personnel or department for questions/problems.
  • Telephone number(s) – a dedicated and secondary number may be given.
  • Fax number(s) – a dedicated and secondary number may be given.
  • Preferred notification method when an automatic notification process is in place. This will be used by our systems to automatically initiate the work completion requests to the appropriate CLEC personnel or department.
  • Email addresses for contacts.
The fax numbers will be used to notify the CLEC of work completion for service orders and repairs. It is suggested that one fax machine be dedicated to Sprint for this purpose and not shared for other activities
Resellers will not need to provide contact information for Local Number Portability nor 911 Record Management.
3) What Services will CLEC Order – Indicate the Service(s) that the CLEC plans to order from Sprint.
4) Contract and State-specific Information - Indicate the state(s) in which the CLEC intends to conduct business and where executed interconnection agreements are in place. Separate OCNs are required for carriers offering both facility-based and resold services. Include OCN numbers and the assigned Telcordia Company Identification Code (CIC). The OCN is required to implement service and to facilitate billing.
If the CLEC chooses to use a CIC, it must be unique from that used by any IXC affiliate.

In addition, provide the LNP Service Provider ID (SPID); a unique code assigned by Telecordia that is utilized for Local Number Portability (LNP) and identifies the switch provider ID where a telephone number has been ported to and from. NENA code is used to lock/unlock 911 records.
5) Is CLEC switch capable of 10-digit trigger (TDT) – Sprint is capable of providing the 10 digit trigger when porting a number out to the CLEC but also needs to know if the CLEC has the capability to provide the 10 digit trigger in their switch when porting a number into Sprint.
