Class List Permission & Donation Request
Dear Parents/Guardians,
We would like your consent in compiling a class list with names, phone numbers and addresses of students in Mr(s).______’s class. Please fill out the form below and return it tonight, or in your child’s backpack by September ____. Class lists are helpful when arranging play dates, class parties, homework help, etc.

The suggested VOLUNTARY donation is $15.00. This amount will cover the cost of class party crafts, as well as teacher’s gifts on behalf of the entire class (during the holidays, teacher’s birthday and at end of the year). Please only contribute what you feel you can.
