·  Please indicate which option you will be taking by filling in the box below and then return this form. Cheques should be made payable to Mineral Products Association, or credit card payments can be made by contacting Mary Burling. If you require an invoice, a PO number must be supplied with this booking form

·  Notification of cancellations must be received in writing at least 7 days prior to the event, no refund will be given after this date unless the place is resold. No refunds will be given to non-arrivals.

·  Substitutes will be accepted at any time.

·  All bookings will be confirmed with an email giving directions and details. If this email is not received please contact Mary Burling to ensure your booking has been made.

Number of tables (max. 10 people) at £980 + VAT [£196] = £1,176 per table
Display Stand Area (includes one free place) at £175 + VAT [£35] = £210
