I understand that my West Port High School student is requesting to bring the above named guest to the Prom on April 1, 2017 to be held at Disney World. The student guest must bring a photo student ID or driver’s license the night of the event in order to attend the Prom. By signing this form, I acknowledge and give permission for the guest to attend the WPHS Prom with my student/child. *I understand that all students must ride the approved charter bus and may not transport themselves.
