●Turn in your completed application by 3:30pm on January 18, 2017 and be entered into a random drawing for an Early Bird Scholarship.
●Only Seniors enrolled in the Coos Bay School District who will receive a diploma/GED within the 2017 school year may apply.
●Applications must be typed.
●Neatness counts! Proofread carefully. Keep the scholarship in the original format.
●The scholarship committee will select the scholarship(s) for which you best qualify.

Instructions
1. Go to yourgoogle account. Join class: rrphlh Make copy of the scholarship application.
Do not make any changes to the original format.
2. Parent/Guardian and Student MUST sign application. (even if student is 18)
3. Application should be put together in the following order, each should be limited to 1 page:
a) Personal and Academic information
b) Family and Financial Information
c) Copy of FAFSA that includes your EFC (estimated family contribution) (or ORSAA)
d) **Optional** Special Financial Consideration
e) Copy of Activities Chart - from OSAC or typed copy of the provided PDF
f) Copy of Personal Statements 2 per page. Include your name, question and number
g) **Optional** Include any college transcript you earned credit from
h) **Optional** Jason Forrester or Vocational/Technical Scholarships info page
i) **Optional**Recommendations for Forrester (2) or Voc/Tech (1) Scholarships
4. Staple the entire package (do not include this cover sheet) left hand corner

Personal Statements
Each statement is a maximum of 1000 characters (approximately 150 words)
Let your Personal Statements tell your story. Donors are looking for reasons why they should choose you for their scholarship award. Show the “unique you” in your Personal Statements and use correct grammar, punctuation, and spelling. Don’t repeat information already included in your application, such as that your greatest accomplishment is your GPA. Focus on something unique about yourself! For all Personal Statements, pay attention to the topic and any length requirements.
Please insert two statements per page and include the prompt. Make sure your name is on the top of each page. Use a minimum of 14 font. Do not include this page when you turn in application.
1) What are your specific educational plans and career goals and why? What inspires you to achieve them?
2) What have you done for your family or community that you care about the most and why?
3) Describe a personal accomplishment and the strengths and skills you used to achieve it.
4) Describe a significant change or experience that has occurred in your life. How did you respond and what did you learn about yourself?

Marshfield Vocational/Technical Scholarship. These scholarships are awarded to students planning to advance his/her education and training in skills needed for a career in a vocational or technical trade that requires a certificate or an associate’s degree. For example: electrician, carpenter, heavy machinery operator, line school, auto technician, truck driving school, EMT, welding, cosmetology, medical or dental assistant, pharmacy technician, culinary arts, etc.
Students pursuing a bachelor’s degree do not qualify for these scholarships.
Selection criteria include:
· Motivation
· Leadership
· Involvement in school/community activities and service
· Financial need is a consideration
* The committee will pay particular attention to the applicant’s goals and plans for achieving them in the personal statement, question #1. Please be specific as possible.
* One reference from a teacher or an adult mentor is required. The recommendation forms are available online.
* Applicant must complete an interview with members of the voc/tech scholarship committee, which will be scheduled in the spring by the committee.

Jason Forrester Memorial Scholarship. This scholarship honors the memory of Jason Forrester, a former MHS Student. One award of $1500.00
Selection criteria include:
· Lettering in a competitive sport
· Good moral character
· Demonstrated leadership
· Minimum 3.00 GPA
*The committee will pay particular attention to the applicant’s goals and plans for achieving them in the personal statement, question #1. Please be specific as possible.
* Two references from adults in the community are required to be considered. One recommendation may come from a Coos Bay School District staff member, the other must be an outside community member. The recommendation forms are available online.
