MEN & WOMEN OF EXCELLENCE

CRITERIA

Please be advised that each of the following criteria must be met in order to consider your application for the AHAINA Man or Woman of Excellence Award. All material must be submitted by 5:00 p.m. Wednesday, March 18, 2009 to AHAINA Student Programs, Rasmuson Hall, Room 106.

Minimum Requirements

Candidate must be an undergraduate junior or senior, formally admitted to UAA, currently enrolled in 6 or more credits, and have a cumulative GPA of 3.00 or better.

1.  Official application form.

2.  A 250-300 word essay describing the most meaningful and stimulating achievement in your field of study and how your experiences relate to your long range goals. (Essay should be typed and submitted with cover sheet including your name and essay topic).

3.  Three (3) letters* of recommendation documenting your performance/ participation in each of the following areas:

a.  Classroom/academic – Professor, Department Chair or Dean

b.  Campus organizations/activities – Advisor or organization president

c.  Community services – Volunteer Coordinator

4.  Unofficial Transcript.

5.  Copied documentation of accomplishments/achievements during your college career: certificates, citations, newspaper articles, publications, etc. DVDs and slides are also acceptable. Your portfolio will be returned after the Awards Ceremony.

6.  Submission of a wallet-sized photograph (black and white or color) for public relations. If a professional photo is used, please provide a release form from the photographer.

*Letters must be on official stationery.
