**Milk will be served with every Hot Lunch.
If for some reason your child does not want milk, please send a drink that day.**

Please complete the order form attached: Circle the amount for those months your child would like to receive hot lunch. Place an X over any dates your child will not be ordering lunch. If necessary, the committee reserves the right to replace menu items with a similar district-approved item.

If you are able to volunteer for any or all of the above dates, please fill out the parent volunteer form below and return it with your child’s hot lunch form. Your participation is greatly appreciated!!

Return the attached hot lunch form, the parent volunteer form and your check (payable to: Summit Hill School District 161) in an envelope clearly marked with your child’s name, grade and homeroom teacher’s name no later than Thursday, September 3, 2015. Checks only please!

Hilda Walker Hot Lunch Parent Volunteer Form 2015-2016
If you are interested in volunteering for any of the dates, please fill out the form below. You will be contacted via email regarding your availability prior to each hot lunch date. Phone calls will be placed only if necessary. All volunteers will need to fill out a Volunteer Form prior to volunteering (please see the district website for the volunteer form). On the day of Hot Lunch, volunteers will need to sign in at the school office (with a photo ID) and report to the lunchroom kitchen. Volunteers are needed beginning at 10:30am and ending at 12:15pm. Please remember that siblings are not allowed.
