REQUIRED PROCEDURES FOR ADVANCED-LEVEL AFFILIATED TRAINEESAT EISENHOWER ARMY MEDICAL CENTER, FORT GORDON, GEORGIA
1. Access To Fort Gordon: Trainees who do not possess Department of Defense identification will be required to go to the Visitor’s Center at Gate 1 and will receive a 1-day entry pass - a valid drivers license, vehicle registration and proof of insurance will need to be presented. Fort Gordon now requires a full background check for access for more than one day.PS Form 190-6, signed in the original, with copy of driver’s license and Social Security Card, should be presented to the Office of Graduate Medical Education for processing and once received by Fort Gordon Provost Marshal staff a 5-day pass will be granted. When the background check is completed a 30-day pass is possible.
2. Eisenhower Security Procedures: It is required that all trainees have a record of their fingerprints on file to enable security screening by the Eisenhower Security Officer prior to inprocessing. Those who wish to rotate for longer than 120 days are subject to more stringent requirements and should contact the Graduate Medical Education Office for further information. In order to pre-screen for the security check, the EAMC Graduate Medical Education Office (GME) must haveattached form “Rotation Request Personnel Information” completed for all trainees a minimum of 60 days prior to the rotation start date. If a trainee was previously screened within the past 12 months for a rotation at a Veterans Affairs facility, please let us know in advance.We regret we cannot accept non-U.S. citizens for training.
Trainees requiring fingerprinting should visit the EAMC Security Office,Trailer 3 (across from the main hospital) Monday to Friday from 0730 to 1130 hours only (Not Thursdays). This is a walk-in service, appointments are not available. Fingerprinting should be accomplished no later than 30 days prior to the requested start date. Failure to comply with this advance time limit will result in delay of start of rotation.
2. Immunizations: All trainees are required to have the immunizations as listed on the attached form and to complete theform appropriately for review by our Occupational Health Section. They may visit Occupational Health as walk-ins prior to their rotation start date; or, if you have a contact at DDEAMC, the form may be emailed to them in advance; or, on the day of inprocessing, students will hand-carry it to Occupational Health as part of inprocessing. Questions regarding immunizations should be directed to the Occupational Health Section, 706-787-5124.
3. HIPAA Certification: Trainees must bring documentation to verify that they have received training or re-training regarding the HIPAA laws. This must be currentwithin the year. If this documentation is not available, trainees will be required to obtain the training prior to beginning their rotation.
5. Reporting for your Rotation: Report to the Office of Graduate Medical Education, Room 4B02, after 8 a.m. on Monday (if Monday is a Federal Holiday, report on Tuesday). Contact the GME Office, 706-787-4657 if you have questions on reporting.
