We offer many activities: creative workshops, advocacy training, volunteering opportunities, organised healthy walks and much more. Registerwith usfor a 45 minute meeting to complete an individually tailored ‘Wellbeing Plan’ with a Wellbeing Advisor. We will explore your interests, identify goals and/or areas of your life that you might like support with.

- Improving Hospital Discharge Summaries
Introduction
King’s is making efforts to improve the patient experience of discharge from hospital and to improve the quality of the GP discharge summary
Main story
We have undertaken an audit of the quality of discharge summaries across a number of surgical and medical specialties. The audit was led by GPs across Lambeth, Southwark and Bromley with input from hospital clinicians.
Some of the quality themes highlighted were:
- Lengthy summaries that were difficult to follow
- Lack of clarity about follow-up arrangements
- Change in medications and the rationale
- Requests for GPs to chase diagnostic results (hospital responsibility)
- Frequent use of acronyms/ abbreviations
- Unclear or absent discharge plans for packages of care to be provided by community and social care providers
- Lack of information given to patients and carers about diagnosis, treatment and on-going care
- Lack of hospital baseline results which need on-going monitoring in primary care e.g. BP, HbA1c, Hb
The findings are being shared with all hospital consultants and with the multidisciplinary teams that organise discharge arrangements so that we can plan to improve the quality to make them more concise and informative. We hope you will see an improvement over the coming weeks and months.

- Communication from King’s Hospital
A new CQUIN initiative has started at Kings College Hospital NHS Foundation Trust to document and communicate to GPs, patients’ medication reviews undertaken during their admission. This project is being led by Professor Steve Jackson, Consultant Clinical Gerontology.
Patients (from Lambeth, Southwark and Bromley) discharged from a geriatric medicine ward (Byron, Donne, Marjorie Warren and Mary Ray wards at Denmark Hill; S1 and S2 wards at the PRUH) will be discharged with a Clinical Medication Review letter (CMR), which will give GPs detailed information on medicines stopped, started, changed and any follow up recommendations. It will be sent with the discharge summary and written by the pharmacist and doctor responsible for the care of the patient. A copy will also be given to the patient on discharge to give to their community pharmacist.
When you receive the CMR, please consider the recommendations and amend the medication records as appropriate. We hope this initiative will ensure that information about medicines is effectively transferred and will improve the experience and continuity of care for our patients. Please let us know if you have any concerns over our recommendations.

- SEXUAL HEALTH IN PRACTICE (SHIP)
This interactive, practical, peer-led training in sexual health has been tailored for GPs and Practice Nurses in the primary care setting. It will enable LSL clinicians to update knowledge and develop further skills in assessing sexual health risk, by offering real life approaches to HIV and STI testing, diagnosis and management. Join the 240 LSL clinicians who have benefited so far from SHIP in changing their practice to tackle our significant local HIV & STI rates
