INVITATION TO JOIN PATIENT REFERENCE GROUP

I am writing to let you know about an exciting opportunity to be involved in the decisions that are made about the services we offer here at Fordington Surgery.

A number of years ago we had a Patient Involvement Group which met on a regular basis to discuss issues around service provision, premises and the day to day running of the surgery. Unfortunately this group disbanded a few years ago due to a drop in numbers. We would like to re-establish this group but in a more up to date form. We propose to create an on-line forum whereby patients can join up using an e-mail address, receive surgery updates and relevant questionnaires relating to real issues around patient care and be involved in some of the decision making within the practice on-line.

We would very much value your involvement in this project and hope that you consider it to be a worthwhile use of your time.

This will be sent to me as Practice Manager and I will add you to our on-line forum. In due course you will receive correspondence from the practice about issues we would like your opinion on. You are also able to submit your own comments and suggestions for consideration by the group.
