2017 China Study Tour Application Form

Terms and Conditions

·  This program is open to CSDE full time students and school-based Chinese students only.

·  The participants must be born in or before 2002.

·  The study tour organising teachers may select participants based on the date received the application and deposit, students’ overall academic result, behaviour and attendance record.

Eligibility

To be eligible to apply for the 2017 China Study Tour, the applicant must:

·  Be a full time CSDE student, or a school based student in Chinese program

·  Be in Year 9 to Year 12 and born in or before 2002

·  Be in good health and fit for international travel

·  Be prepared to abide by the Code of Conduct for Students

·  Purchase travel and medical insurance as per minimum requirements identified by DET

·  Be prepared to abide by the laws of China whilst in Mainland China and Hong Kong

·  Be willing to participate in all activities arranged by the school

·  Have (or be willing to obtain) a current and valid passport (with at least 6 months validity from 14 September 2017)

·  Participate in pre-departure briefings which will be conducted online

Instructions for completion of the form

1.  Parent and student information night – 24th October 2016

2.  Student complete and submit the application form and deposit $500 – 24-28th October 2016

3.  2nd parent and student information night – 16 June 2017

4.  Submit medical and travel insurance, visa application form, immunisation certificate, Code of Conduct for students, passport original copy, 2 colour passport photo – 16 August 2017

5.  Final parent and student information night – 28 August 2017
