Applications are submitted to the Chapter Advisor or Colony Advisor who will forward it to the National Office.

PHOTOGRAPHS:

Please submit a quality photograph of yourself that will be published in The Compass of Theta Phi Alpha should you receive the award. The best quality photographs would include your individual composite photograph or a senior or graduation portrait. Otherwise, a clearly focused head and shoulders photograph should be sufficient. When selecting the photograph for submission, please insure that the photograph has a light background and that nothing seems to be coming out of your head or shoulders, such as a window frame or bulletin board. If you need to submit a temporary photograph at the time of application, then a quality photograph later, that is acceptable. Your quality photograph must be submitted to National Office no later than January 15.

SUMMARY STATEMENTS:

Please submit a one or two page summary, which provides additional information that should be considered for this award. This could include such items as activities or honors not addressed by the categories of this application, accomplishments in office, and/or important moments as a sister of Theta Phi Alpha.

Dear Member of the Academic Community,

The Senior Service Award of Theta Phi Alpha Fraternity is bestowed upon a young woman whose scholarship, leadership, character, and service to our fraternity and her school have been outstanding during her years as a collegian. The Advisory Board of each Theta Phi Alpha chapter evaluates all applications and supporting documents to nominate a maximum of two women from each chapter for submission to our Board of Trustees. The Board of Trustees then selects no more than one woman from each chapter to receive this prestigious award.

As support for the collegians’ application, the Board of Trustees requires each applicant to provide a letter of recommendation from a professor or administrator at her college or university. You have been chosen by the applicant to provide a letter on her behalf. Letters of recommendation should comment on the applicant’s service to her school as well as on her character. Please include the length of time you have known the applicant and in what capacity.

To ensure the confidentiality of our selection process, we ask that your letter be given to the applicant in a sealed envelope with your signature across the seal. The applicant will include your sealed envelope with her application when it is submitted to her chapter’s Advisory Board. Alternatively, you can submit your letter of recommendation electronically directly to the Advisory Board using the Chapter Advisor’s contact information provided below or by sending your letter as an email attachment to the following address: . The alumnae members of the Advisory Board will read all letters as they narrow their selection to the two nominees. Collegians have no knowledge which applicants become the Advisory Board award nominees.
