CHECKLIST FOR OBTAINING A LICENCE FOR JAN AUSHADHI NEW STROES
1. Forwarding letter containing details of all enclosures.
2. Duly filled Form 19 (2 copies) signed by authorized signatory.
3. Original copy of Challan. Rs 3000 fee to be deposited in the scheduled bank.
4. Authorization of the Rogi Kalyan Samiti in favour of the person submitting the application and responsible for conducting the business.
5. Copy of resolution passed by Rogi Kalyan Samiti for opening of Jan Aushadhi store.
6. Copy of constitution of Rogi Kalyan Samiti.
7. Copy of Registration of Rogi Kalyan Samiti with the Registrar Co-operative societies.
8. Declaration by Rogi Kalyan Samiti that no case of any dispute of Jan Aushadhi is pending in the court of law at the time of filing of the application.
9. Copy of approval of the Govt. for opening of Jan Aushadhi store in the proposed site of the hospital.
10. A complete undertaking in the form of affidavit to be submitted by authorized signatory for running the Jan Aushadhi store.
11. A complete undertaking in the form of affidavit by the Pharmacist appointed by the Rogi Kalyan Samiti for the purpose of sale of drugs.
12. A agreement of Civil Surgeon/SMO of the hospital for the leasing out of proposed site as per policy of Jan Aushadhi concept.
13. Copy of rent/lease of current month.
14. 2 identical copies of site plan of the proposed site signed by the approved architect and the applicant.
15. Copies of the appointment letters and joining reports of qualified pharmacist appointed by the Rogi Kalyan Samiti.
16. Copies of bills/receipts of refrigerators and ACs etc.
17. 3 copies of ID card of each qualified person.
NOTE: All photocopies shall be attested by Gazetted officer with his name and designation or by Notary.
STEPS TO BE TAKEN BY DEPUTY MEDICAL COMMISSIONERS/SENIOR MEDICAL OFFICERS
1. Resolution to be passed by Rogi Kalyan Samiti, expressing willingness to run JAS.
2. Rogi Kalyan Samiti to be got registered from Registrar Co-operative Societies by depositing required documents and fee.
3. Identification of site for Rogi Kalyan Samiti.
4. Renovation/ Furnishing of shop for JAS.
5. Once Rogi Kalyan Samiti is registered, 3 Pharmacists to be appointed at a salary of Rs 7500 p.m for each pharmacist by inserting advertisement in the leading newspapers.
6. Interview to be called.
7. 10 days time to be given as joining time for Pharmacists.
8. Drug Licence to be applied in the name of Chief Pharmacist (one of the pharmacist may be designated as Chief Pharmacist) as per checklist provided by Assistant State Drug Controller (attached).
9. Meanwhile JAS site would be ready. Tie up with CPSUs for supplies, Purchase of Computers, software, Refrigerator, AC etc.
