You need to provide original documentation in one of the ways described below. Please use the diagram below to determine which of these documents you need to provide.

If applicants cannot provide documents as stated above, further identity checks will be made. This will delay employment and will incur a cost. Please contact HR Direct if documents cannot be provided.
How do applicants have their documents of identity checked?
1.By taking their documents to their Line Manager or Headteacher
The Line Manager or Headteacher where you work should then take a photocopy of the documents, sign and print the copies and return to us along with your completed DBS application form.
- Come into County Hall
You can visit us in County Hall, Wynnstay Road, Ruthin between 9.00am – 5.00pm Monday to Thursday, 9.00am – 4.30pm on Friday and we will check your documents in person.

- Current UK Driving Licence (photocard or old style paper version)
- Birth Certificate (UK and Channel Islands) issued after the time of birth
- Marriage / Civil Partnership Certificate (UK and Channel Islands)
- HM Forces ID Card (UK)
- Fire Arms Licence (UK, Channel Islands, Isle of Man)
- Current non-UK Photo Driving Licence
Group 2b
- Mortgage Statement ** (UK or EEA)
- Bank / Building Society Statement * (UK or EEA)
- Bank / Building Society Account Opening Confirmation Letter (UK)
- Credit Card Statement * (UK or EEA)
- Financial Statement ** e.g. Pension, endowment, ISA (UK)
- P45 / P60 Statement ** (UK and Channel Islands)
- Council Tax Statement ** (UK and Channel Islands)
- Work Permit / Visa ** (UK)
- Letter of sponsorship from future employment provider – non-UK or Non – EEA only
- Utility Bill * (UK) not mobile telephone bill

- Central or local government, government agency, or local council document giving entitlement – eg from the Department of Work & Pensions, the Employment Service, HMRC – UK and Channel islands

- Cards carrying the PASS accreditation logo (UK and Channel Islands)
- Letter from Head Teacher or College Principal 16/17year old in fulltime education (only used in exceptional circumstances when all other documents have been exhausted)

An Applicant’s Guide to Completing the
DBS Application Form
DO’S
- Use black ink and write clearly in BLOCK CAPITALS only.
- Use only one letter or number for each box.
- To correct a mistake on the form, put a line through it and write the correction clearly to the right. If there is no space on the right, write the correction as close as possible.
- Leave an empty box between words, but not between postcode or telephone numbers
- Mark choices in the boxes indicated with a cross (X), not a tick
- Be sure to sign the declaration at Section E, item 56 keeping all of your signature inside the box.
DON’TS
- Don’t write over the edges of the boxes
- Don’t place any stamps or stickers on the form, i.e. featuring addresses or dates.
- Don’t strike out a section of the form or state a field is not applicable. If it is not relevant to the application then please leave it blank.
- Don’t staple any attachments to the form.
- Don’t use correction fluid.
- Don’t complete Sections D, W, X, Y and Z.
- Don’t return the form to the DBS.
- Don’t forget to have your identity checked in one of the ways shown overleaf. Your DBS application will not be processed until your documents have been checked.
