The Registration Fee is $100.00. Multiple player discounts apply: 1st Child- $100.00, 2nd Child - $90.00.

The Grid Kid Football organization will furnish to the player: a helmet, and shoulder pads. The league will also provide footballs, referees, and accident insurance. The participants must furnish pants (black), pads, a mouth piece, athletic supporter, and appropriate shoes (molded rubber cleats of soft composition in accordance with the National High School Rules -- No metal, or metal tipped, cleats are allowed!)
Practices will start on August 1, 2016. The individual “team coaches” will inform the players of the practice time and location, and time assigned for their team to receive equipment. No equipment will be issued prior to the specified time for the specific team. No registration fee refunds will be given after August 15, 2016. No team or coach is allowed to start practices prior to August 1, 2016.
Physical examination/doctor certification is required to play GRID Kid Football. A physical is required if this is the player’s first year of playing Idaho Falls Grid Kid Football.
Copies of the Doctor approved/signed physical form must be provided to the league with this registration form.

Parental Approval:
I give my approval for my child to participate in Grid Kid Football and release all officers, coaches, and other league personnel from any liability in the event of injury or accident. I agree to maintain the league issued equipment and return it in good condition at the end of the season. If any piece of league-supplied equipment is purposely altered or modified while in possession of the player, I will reimburse the league for the replacement cost of such equipment. I also agree to abide by all rules outlined in the League Rules, Regulations and By Laws of Upper Valley Grid Kid Football.
