CHAPTER I

THE PROBLEM AND ITS BACKGROUND

Therapeutic Communication in Nursing reinforces the nurse-patient relationship. It makes the nurse appear more humane to a patient. It can help cut through barriers of culture and gender, establish a connection if there was a breakdown in communication and help deal in a situation where empathy is needed with the patient. With therapeutic communication, the patient’s emotional state is considered as well as their feelings. Being sensitive to the needs of a patient and their turmoil is very important. A lot of nurses do not understand this and do not know how to deal effectively with a patient as a result.

According to Peplau’s Interpersonal Relations Model the nurse-patient relationship evolves in four phases: 1.) Orientation. In this phase the nurse must earn the trust of the patient; 2.) Identification. During this phase the nurse’s focus is to assure the person that the nurse understands the interpersonal meaning of the patient’s situation; 3.) Exploitation. During this phase the patient learns to use available services based on self-interest and needs. 4.) Resolution. In this final phase old needs and goals are being resolved and new problems are adopted.

Based on the preceding statements, the researchers want toprove that the age really matters in terms of perception and satisfaction.

STATEMENT OF THE PROBLEM

This study aimed to determine the “Influence of therapeutic communication of staff nurses to recovering patients in government hospital”.

Specifically it aims to answers the following questions:

  1. How powerful is therapeutic communication in affecting the recovery of the patient?
  2. What is the reason why therapeutic communication is not being practice?

HYPOTHESIS

h.oThere is no significant difference between the use of therapeutic communication on patients to their recovery.

h.1 There is significant difference between the use of therapeutic communication on patients to their recovery.

ASSUMPTIONS

The proponents of this study assume the following:

1. Therapeutic communication as an independent nursing function will help patients improve their health status thus fastens their recovery.

2. Recovering patient will enhance their emotional well being and helps reinforce nurse-patient relationship.

3. Staff nurses will understand the importance of therapeutic communication to the continuous recovery of the patient as well as to establish this in providing health education.

SIGNIFICANCE OF THE STUDY

In particular, this is significant to the following target populations:

To the Patients,

This study will be very beneficial to them, as they are the primary subjects in this study. This will give them an opportunity to minimize their anxiety that can lead them to fast recovery.

To the Nurses,

This study will make them value the importance of therapeutic communication for the patients hence, promoting faster recovery and a better relationship with them.

To the Student Nurses,

This study will be beneficial to them because as early as now, they will enable to expand their knowledge and they can already apply their learnings on their future profession.

To the Future Researchers,

Finally, this may serve as a significant reference for future researchers that will further improve this study.

SCOPE AND LIMITATION

The problem under investigation was to determine how therapeutic communication influenced the recovery of the patient. This aims to describe the effectiveness of the use of therapeutic communication of staff nurses as a nursing function to fasten recovery of the patients.

This research is designed to present the concept of therapeutic communication to influence staff nurses function with regards to interaction to help in modifying the illness and providing health education to patient under recovery stage.

In line with the focus of this study, the respondents are limited to patients under _____ in selected _____particularly in ______who are in recovery stage.

Patients limited to ____persons, age of 30-60, a Filipino either male or female. Gathered data will contribute to answer questions how therapeutic communication influenced the recovery of the patient.

DEFINITION OF TERMS

Therapeutic Communication. Is an independent nursing function that is use to modify illness and to provide health education.

Patients. The one who receives medical treatment and evaluates the services that is being rendered by the staff nurses.

Nurses. Health care professionals that provides care and initiates therapeutic communication that will lead to patients recovery.

Therapeutic Communication is an interaction that is helpful and healing for one or more of the participants; the client benefits from knowing that someone cares and understands and the nurse derives satisfaction from knowing that he or she has been helpful to someone. The goal of therapeutic communication is to help clients talk about and resolve their feelings and problems related to health, illness, treatments and nursing care. (Craven & Hirnle, 2007)

Therapeutic communication involves used of carefully selected communication interventions to help patients and families overcome stress and adjust to the unalterable. (Schuster, 2000 p.7)

THEORETICAL FRAMEWORK

According to Peplau therapeutic communication focuses on the interpersonal processes and therapeutic relationship that develops between the nurse and client. The interpersonal focus of Peplau’s theory requires that the nurse attend to the interpersonal processes that occur between the nurse and client. Interpersonal process is maturing force for personality. Interpersonal processes include the nurse- client relationship, communication, pattern integration and the roles of the nurse. Psychodynamic nursing is being able to understand one’s own behavior to help others identify felt difficulties and to apply principles of human relations to the problems that arise at all levels of experience. This theory stressed the importance of nurses’ ability to understand own behavior to help others identify perceived difficulties.

This diagram shows that before we attain satisfaction by means of therapeutic communication there are certainbarriers that we encounter during the interaction. The nurses experience stress during their stay in the hospital area for the reason that the client needs different kinds of care according to their present condition. Most of the time nurses cannot perform their responsibilities as a health care provider because of existing barriers. On the other hand patients also experience certain barriers as shown in the diagram. This illustrates that the barriers depends on the clients existing environment and emotions.
