Job Description:The Assistive Technology (AT) Coordinator is responsible for working with all age and types of people with disabilities in assessing, recommending, and writing up evaluations of Assistive Technology (Wheelchairs, walkers, bedside commodes, shower benches, power chairs, grab bars, porch lifts, stair glides, ramps, porch lifts, etc.) and home modifications (widening doors, remodeling bathroom, kitchen, laundry rooms to be more accessible) so that individuals with disabilities can live more independently in the community.

The successful candidate will be able to manage time effectively and be able to write up their evaluations/assessments and be knowledgeable in all aspects of Assistive Technology and necessary home modifications. As well as be motivated to seek out additional funding/apply for funding to get/complete AT equipment and or home modifications requests. Must be creative and use imagination to develop ideas to design devices to assist a person with a disability to live independently.

Assist in supervising the Loan Bank Specialist and work with AT Coordinator to complete AT applications to funders and be a backup van driver for the Center’s 14 passenger van.

Job Duties:

  • Visit with individuals with disabilities in their homes or places of work to assess their various AT/Home modification needs
  • AT evaluations include but not limited to:
  • Completing wheelchair assessments and recommending models/types
  • Design and draw plans for ramps/porch lifts
  • Complete assessments and develop plans for stair glides
  • Asses’ individual’s needs for daily living (shower chairs, writing aids, door openers, bathroom aids, kitchen or cooking aids, etc.)
  • Develop home modification recommendations (widening doors, bathroom redesigns, kitchen redesigns)
  • Work with the various vendors/insurances to get bids and purchase AT
  • Responsible for completing written evaluations for AT needs
  • Develop and maintain relationships with vendors and insurance
  • Coordinate and monitor equipment completion by follow up with individuals once they receive AT to insure that the correct equipment was purchased and installed, and that they know how to use the AT properly, and that the AT meets their independent living goals
  • Preforms home safety evaluations
  • Work with individuals on proper use and maintenance of adaptive equipment or devices
  • Interacts with individuals in a professional manner with respect for their rights and maintain adherence to the independence living philosophy
  • Maintain knowledge of the newest equipment available
  • Attend seminars, and continue training for new equipment as it becomes available
  • Advocate on issues promoting for independence of people with disabilities
  • Work with Loan Bank Specialist to maintain inventory in AT Loan Bank
  • Assist with Loan Bank
  • Clean and keep AT equipment in good working order;
  • Deliver, install, and pick up AT as needed and applicable;
  • Maintain records of all consumer contacts and transportation records;
  • Work throughout the community in educating agencies, organizations and community members of the availability of services offered by Roads;
  • Be a backup driver to provide necessary transportation for Roads Consumers for community integration activities, classes and peer support activities in the community;
  • Secure passengers with accessible equipment for safe transportation to and from activities;
  • Attend staff meetings and participate in general center events;
  • Perform other duties as assigned by the Executive Director.

Requirements:

  • Must be customer friendly;
  • Knowledgeable about disabilities
  • Must knowable about AT(Wheelchairs, walkers, bedside commodes, shower benches, power chairs, grab bars, porch lifts, stair glides, ramps, porch lifts, etc.)
  • Have good writing skills
  • Must have good computer skills (Word, Outlook, and Excel would be a plus)
  • Have good inventory skills
  • Must be able to lift at least 50+ pounds
  • Must be able to do heavy lifting, squatting, bending, turning, hauling
  • Must have knowledge on trouble shooting repairs on AT equipment
  • Must be able to drive Center’s 14 passenger van
  • Reliable transportation to travel to and from work, throughout the surrounding areas
  • Must have an acceptable Criminal Background Check
  • Must have a valid Utah Driver’s License, proof of insurance and good driving record.

Preference given to those that possess:

  • Knowledgeable about the different types of health insurance (Medicaid, Medicare, Healthy U, Molina, Select health, etc.) when it comes to covering AT
  • High school graduate or GED, some secondary education preferred;
  • Familiarity with disability issues and independent philosophy a plus;
  • Knowledge of the ADA a plus;
  • General knowledge of construction and/or small motors a plus;
  • Bilingual a plus;
  • Person with a disability plus;

Working Conditions:

Work schedule requires dependability to complete tasks in a timely manner and be motivated to reach out to the community at large to develop resources and connections to assist people with disabilities in living independently in the community. Work is performed in an office environmentally controlled setting and in the community and at various individuals homes and places of work. Tasks are often performed for sustained periods outdoors and occasionally in hot, cold, or inclement weather. The position requires traveling out of area and working flexible hours, rarely includes weekends and evenings.
