Care Coordination for Children Program

Purpose: Participant or, as applicable, Participant’s Participating Entities listed on Exhibit D are local care management entities (LCMEs) organized for the delivery of community-based care management and have entered into a written agreement with one or more CCNC Networks to deliver care management services to the Medicaid population under a care coordination for children (“CC4C”) initiative that will positively impact the quality and cost of health care for Medicaid beneficiaries between the ages of 0 and 5 (“CC4C Service Agreement”). Pursuant to the terms and conditions in this Agreement and the CC4C Service Agreement, Participant, or, as applicable, Participant’s Participating Entities, agree to utilize CCNC’s Case Management Information System (CMIS) and Informatics Center applications (e.g., IC Reports, Provider Portal) to maximize its collaboration with NCCCN and applicable CCNC Networks to achieve performance goals and benchmarks through the use of best practices and adherence to program standards.

Roles and Responsibilities of NCCCN: NCCCN is a population management program that provides clinical and budgetary coordination, oversight and reporting. NCCCN also provides Participants and Participating Entities access to health information technology, health information exchange, and reports and analytics.

Roles and Responsibilities of CCNC Networks: CCNC Networks provide support, including program oversight and administration, coordination and collaboration for local care management entities to carry out the expectations of the CC4C Service Agreement and achieve program goals. For specific duties related to the CC4C program, see Section II of the CC4C Service Agreement.

Roles and Responsibilities of Participant and Participating Entity: Participants and Participating Entities that are parties to the CC4C Service Agreement are expected to collaborate with NCCCN, CCNC networks and pediatric providers to develop and utilize evidence-based and best practice approaches to provide care management to the early childhood population through the CC4C program and to adhere to program expectations.
