I Have Read, Understand and Agree to the Following

PARENT AGREEMENT 2017-2018

I have read, understand and agree to the following:

To pay a registration fee of $145 for first child

$115 for each additional child/$115 for family class $30 for each additional child

To pay first and last month’s tuition by orientation in September.

To pay a monthly tuition fee for the class which my child is registered

I agree to pay the full tuition regardless if my child is able to attend all class days or not

•I will attend Orientation, any board meetings applicable to my committee and participate in voting in April for incoming board members.

•I will log at least 2 hours per month working on my committee job responsibilities and other preschool related activities.

•I will help with prescheduled weekly cleans, one quarterly deep clean and the end of year deep clean at the Preschool.

•I will come to school on my workday. If I am unable to attend, I will make every effort to find a substitute and notify my Parent Leader. I understand excessive unexcused absences on my workday will be dealt with at the Board's discretion.

•I will commit my time to prepare for and participate in the annual fundraising activity.

•I will serve and actively participate on one standing committee, unless I am serving on the Board of Trustees.

•I will read and abide by the Handbook, Standing Rules and Bylaws of the Cooperative.

•I will keep my child home from school when signs of illness are present (as described in the Handbook).

•I will pay tuition and fees when scheduled. I will reimburse the Preschool for any fees charged to the Preschool by a bank due to returned checks. Late fees of $15 per month will be assessed for any payment not received by the 5th day of the month it is due.

•I will pay a $25 supply fee to DCP.

•I will fundraise $175 for our family or opt out of fundraising and pay $175 directly to Duvall Cooperative Preschool.

In return for my cooperative participation, I can expect the Preschool to do the following for its members:

•Provide activities suitable for the age and developmental level of the child.

•Conduct classes in general accordance with the Riverview School District calendar.

•Provide sufficient adult supervision to insure a positive experience for each child.

•Use educational and safe materials.

Should I be unable to fulfill my responsibilities, I will endeavor to arrange an alternative solution or, if none is found, to give two weeks notice and withdraw from the group. (Both parents should sign this agreement if they will both participate at Preschool).
