Grant Making Advisory for ALL Applicants: Jaguars Foundation Moving to an Online Application System
The Jaguars Foundation is converting our grant application process to an online applications system. ALL applications will only be accepted using the on-line system. You may use the current paper application as a guide, as much of the new application (but not all) will be similar to the current application.
For the first grant cycle of 2012, the new deadline is to be determined. We anticipate the online system to be available by late February or March. We will provide updates on the deadline and status of the new system on this website.
Jaguars Foundation Grant Making Advisory for New Applicants:
PLEASE NOTE THAT NO NEW APPLICATIONS For the First Grant Cycle of 2012 WILL BE ACCEPTED BY THE JAGUARS FOUNDATION UNLESS YOU HAVE FIRST SUBMITTED a one or two page overview of your proposal, well before February 1, 2012, and have received approval by the Foundation’s PRESIDENT to submit a full application.
Your overview should briefly outline the 1) program, 2) nature of the situation and proposed intervention, 3) population and geographic area to be served, 4) total budget, and 5) amount requested from the Jaguars Foundation.
Organizations currently funded by the Foundation that have been invited to apply for continuing funding do not need to submit a one or two page overview before-hand and may proceed to complete the full application. Please be sure to include your Progress Report along with your application.

GRANT PROPOSAL NARRATIVE
Please respond to each of the following using a maximum of 5 typed, double-spaced pages. Suggested lengths for answers are provided at the end of each question. If you are applying for a capital campaign or general operating support, please also attach a long-term (e.g., 3-5 year) strategic plan for your agency.
1. Provide a brief description of your agency’s mission, services, overall size and date founded. (½ page)
2. Succinctly outline the program for which you are seeking funds. Your response should make clear who will be doing what, when, where and for how long (please include program start and end dates). Specific populations to be served should be clearly identified as well as organizations that will be involved as collaborators. (1½ pages)
3. Objectives and Evaluation (1 page)
a) Explain your specific goals for the program and how they will be objectively monitored and measured. Being specific, how will you know whether the program demonstrated success?
b) What do you anticipate your organization will learn from this program that will assist others in future programs? How will the program and what is learned from it enhance your organization’s ability to serve youth and families after the grant period?
4. List what you consider to be the top 3 beneficial aspects of the project as it relates to serving disadvantaged youth. (1/4 page)
5. List what you consider to be the top 3 potential concerns or obstacles of the program as it relates to serving disadvantaged youth, and how you plan to address each one? (1/4 page)
6. How does this program for which you are seeking funding meet the needs of the community it intends to serve, and what evidence is there that the program leaders have engaged the interest and support of the community? (½ page)
7. If the program is to be sustained, provide a reasonable plan for continuity beyond the Jaguars Foundation grant, such as indicating where additional support will likely be sought. (1/3 page)
8. If you have applied for funding from other sources, list these potential sources, amounts you are seeking and when you expect a decision to be made on these requests. (½ page)
9. Board’s Financial Commitment: Describe your agency’s policy regarding expectations for board members to personally contribute financiallyand/or raise financial contributionsfor your organization.

APPLICATION CHECKLIST
The new online application form will indicate items needed which will include:

5. Line-Item Budget with budget narrative for the entire program for which you are applying for funding
6. If your organization is a current grantee and re-applying for this or another program, a Progress Report must be submitted as part of your new grant proposal
7. Only 1 copy of IRS 501 (c) (3) letter determining tax-exempt status. No additional copies are needed.
Note: Do not send the State of Florida Department of Revenue Consumer’s Certificate of Sales Tax Exemption.
8. Only 1 copy of the most recent Audited Financial Statement. No additional copies are needed.
Note: IRS Form 990 should be submitted if the organization does not have an audit.
