The Opportunity
Fellowship Square Foundation is currently seeking an exceptional candidate to join our team as a Resident Service Coordinator for theHunters WoodsFellowship House, located in Reston. This position has the authority and accountability tomanage and coordinate the provision of services related to the Resident Relations Program, including; providing assessments, information, referral to community services counseling, coordination of the services, homemaker assistance, etc.

Full Time and Part Time Position Available

Key Responsibilities

"Resident Relations Program". Consultant will make available to all the residents of the House, in-house social services. The tasks pertinent to the provision of services include:

  • Establish visibility and rapport with the residents,
  • Provide information to residents regarding community services,
  • To act as a mediator, organize, arrange or negotiate contracts with community agencies to obtain services for residents,
  • Attend meetings with community agencies to obtain resource information,
  • Receive referrals regarding residents who may need homemaker support services from the residents, managers, family members, human service professionals,
  • Assess the resident's needs through personal interview with theresident, input from the Administrator, input from the other serviceproviders currently involved with resident (physician, home health agency, community social worker, input from family members),
  • Develop individual service plans to meet resident's needs,
  • Coordinate with residents, Administrator and family members to implement the service plan; Follow-up with the residents periodically to ensure plan is appropriate.
  • The social services referrals may include Social Security, Veterans Benefits, Medicaid, Medicare, food stamps, programs that provide food and clothing, financial assistance, maintenance of housing, immigration issues, meals on wheels, communication, transportation, home health aides, senior centers and pre-emptive health and mental health screenings.

Qualifications

  • This employee must demonstrate the patient, helpful, and courteous demeanor which is essential to good human relations with elderly and disabled persons.
  • Must be able to communicate both orally and in writing;and have ability to maintain confidential resident information.
  • Ability to communicate effectively in English, while in a multicultural environment.
  • Computer knowledge should include Microsoft Word, Excel, and Outlook.
  • Must be approachable as well as flexible and professional with residents, staff, and family members.
  • Good organizational and team work skills. Excellent communication, decision making and problem solving skills.
  • Ability to organize and manage multiple priorities.

Preferred Qualifications

  • Master’s Degree in Social Work
  • Bilingual in Mandarin
