PARENT/GUARDIAN CONSENT FORM

Dear Parent/Guardian:

Your child has been offered the opportunity to participate in The Prince’s Trust Achieve programme, which we are delivering at (insert name of school/centre).

The Achieve programme is developed and managed by The Prince’s Trust (the “Trust”), a national youth charity that helps young people to reach their full potential. It supports 11 to 30 year-olds, including those facing challenges in education. 80% of young people who participated in an Achieve ‘club’ in 2015/16 felt that the programme had increased their education or training opportunities.

The Achieve programme is for young people who are facing challenges in education, supporting them to learn new skills, develop confidence and re-engage with learning so they can reach their full potential.The programme will be delivered by (insert name and/or title of staff delivering) and they will attend an Achieve ‘club’ (insert details i.e. every Tuesday and Thursday, for two hours a week at school).

In order to measure the impact the programme has on the young people participating, The Prince’s Trust will collect some profile data (including contact and diversity information and their opinions about school and challenges they face) when young people register for the programme.The Trust also wishes to collect further data such as attendance and behaviour data about young peoples’ improvement in school throughout their time on the programme. If your child participates in the programme, you can opt out of this additional data collection.

I give permission for my child/ward to be registered for and participate in The Prince’s Trust Achieve programme.

I have read and understood the information provided about my child’s participation in this programme. I agree that any information held about my child by The Trust can be used by The Trust and shared with other organisations working with them, to help run, fund and evaluate the Trust programmes and for the benefit of my child’s welfare, safeguarding or future journey. This information may be kept for as long as required for these purposes.

I give permission for any organisation such as (but not limited to) an employer, voluntary organisation or education/training establishment to pass on information about my child to The Trust for any of the purposes referred to above, including in order to confirm past and present employment or enrolment.
