The Praxis Series®Online Registration E-Ticket and Order Receipt Correction Form

Use this form to make corrections or additions to the information you provided online when you registered. Some of the information below is shown on “Your Order Receipt” only. DO NOT RETURN YOUR E-TICKET. Correction forms must be received at ETS by the date of the test or must be submitted to the test center supervisor on the day of the test.

Correction Form Instructions (Do Not Return Your E-Ticket):

Fill in the identification information as it appears on the electronic admission ticket or order receipt. If there are errors, fill in the correct information in the spaces provided. Name changes will not be made, except for those necessary to correct inconsistent, incomplete, or incorrect information due to spelling errors or reversal of first and last names. Note: Address corrections (only) can be made online or in the space provided.
Adding, Changing, or Deleting Score Recipients – Fill in the code(s) and name of institution as entered online (for changes and deletions) and follow the instructions below.
To add a new score recipient, select “ADD” from the drop-down box, enter the institution’s code number (from the code list on the Praxis® website), and enter the institution’s name in the space provided. (Please note that you may not exceed three score recipients.)
To change a score recipient, select “CHANGE” from the drop-down box, enter the institution’s code number (from the code list on the Praxis Web site), and enter the institution’s name in the space provided.
To delete a score recipient, select “DELETE” from the drop-down box in the space provided, enter the institution’s code number (from the code list on the Praxis Web site), and enter the institution’s name in the space provided.
Some state agencies receive score reports automatically (if you indicate that you attend an institution in that state); however, if your test center is not located in the same state as your attending institution, your score report will not be sent to the state agency unless you list the state as a score recipient.
If you are adding or changing a score recipient, be sure to check the code. If you have not designated three score recipients by the test date, additional fees will be required for each score report requested later.
Please check all score recipient codes carefully. If you fail to correct an error, your scores will be sent to the wrong agency/institution.
Complete this correction form, print, and mail or fax to the address listed below. If you wish to submit this form electronically, complete and save this file to your computer desktop. Compose an e-mail with the subject line “Praxis E-Ticket Correction Form.” Attach this completed document to your e-mail and send it to .
