Information Request

Further to your email of 31 August 2012, under the Freedom of Information Act 2000, requesting information regarding

We are pleased to be able to provide you with the information you requested.

1. How many patients under the PCT were eligible for continuation

of care funding between 2010-2011

559 received full Continuing Care funding.

498 received part Continuing Care funding for delegated health tasks.

2. What percentage (%) of these patients, allocated funding by the

PCT, continued their care by being supported in their own home i.e.

not residential

Approximately 20% of the 559 receiving full Continuing Care funding.

Approximately 99% of the 498 receiving part Continuing Care funding.

3. What was the total expenditure that the PCT allocated to

continuation of care between 2010-2011, for patients who were

eligible, to be cared for within their own home

£3,094,096.00 receiving full Continuing Care funding.

£2,105,631 receiving part Continuing Care funding for delegated health tasks.

4. How does the PCT assist patients in identifying and accessing

the best social care and nursing partners to support the patient in

their own home. What is the process?

The Health Care Manager (HCM) role within the Oxfordshire Continuing Care team is a combined role with responsibility as an assesor for Continuing Care and a HCM to support the delivery of care (see Job description attached).

Once a client is considered eligible for Continuing Health Care funding, a HCM is allocated to the client. The role of the HCM is to arrange for the appropriate care package to be commissioned in order to meet the needs of the client and family.

Private providers are approved and accredited by the Local Authority, the HCM will assist the family to choose the most appropriate for their care. This would be an agency within their local area who had appropriately trained staff with the capacity to deliver the required care package. This may include live in care as well as domiciliary visits.

All financial processes are managed through the Continuing Care department and the finance team at the Local Authority. Internal processes and documentation is completed by the HCM and signed off by the relevant Manager with financial responsibility.

The HCM will support the family with any variation to their care package, agreeing with the agency what is required and gaining authorisation for any increase from the Manager with financial responsibility.

Personal Health Budget (PHB)

In 2010, Oxfordshire Primary Care Trust commenced a Pilot supported by the DH, to introduce PHB’s. This process of personalised care is now part of the service offered to all Continuing Care funded clients and their families.
