Part B. Lesson Plans for Learning Segment (no more than 4 pages per lesson; submit 3-5 lesson plans in 1 file; within the file, label each lesson plan (Lesson 1, Lesson 2, etc.); all rationale or explanation for plans should be written in the Planning Commentary and removed from lesson plans.
Part C. Instructional Materials (no more than 5 pages of KEY instructional materials per lesson plan; submit materials in 1 file; within the file, label materials by corresponding lesson (Lesson 1 Instructional Materials, Lesson 2 Instructional Materials, etc.); order materials as they are used in the learning segment; insert a translation immediately after each instructional material for portions in Braille or in a language other than English).
Part D. Assessments and/or Data Collection Procedure (submit assessment instruments and/or a description of data collection procedures, as appropriate for the lesson objectives measured, in 1 file; within the file, label assessments/procedures by corresponding lessons (Lesson 1 Assessments/Procedures, etc.); for assessments/procedures used in multiple lessons, order and label by lesson when first used; insert a translation immediately after each assessment for portions in Braille or in a language other than English).
Part E. Planning Commentary (no more than 11 pages, including prompts; use Arial 11-point type; single space with 1” margins on all sides; respond to prompts before teaching the learning segment).

Video Clips (no more than 20 minutes total running time; before recording video, obtain permission from parents/guardians of learners (or, if appropriate, the learners) and from adults who appear in the video; refer to Task 2, What Do I Need to Do?(pg25) for video clip content and requirements; when naming each clip file, include the number of the lesson shown in the video clip).
Part B. Instruction Commentary (no more than 8 pages of commentary, including prompts; use Arial 11-point type; single space with 1”margins on all sides; see page 39 for additional information for submitting documentation).
Task 3. Assessment

pp. 40-44) / Part A. Completed Final Assessments
For written final assessments: use correction fluid, or a marker to mask/remove students’ name, your name, and school name before copying/scanning any work samples; if learner’s writing is illegible, write a transcription directly on the work sample.
For video final assessment: if more than one learner is visible, indicate in the Assessment Commentary how the focus learner can be recognized.
Final Assessments: submit 1 copy of a work sample for the primary learning target; submit 1 copy of the completed final assessment for the supporting or secondary learning target (if different from the final daily assessment record and work sample); label the files as Work Sample for Primary Learning Target and Final Assessment for Second Learning Target.
Important: Do not submit an additional file for Part A if the work sample or final assessment for the supporting/secondary learning target is from the video clips from the instruction task or in the final daily assessment record. Provide a time-stamp reference (no more than 2 minutes) in Assessment Commentary.
Part B. Completed Daily Assessment Records and Baseline Data
Daily Assessment Records (submit 1 copy of the completed daily assessment record for the focus learner for each lesson for both learning targets in 1 file; within the file label each record (Lesson 1 Daily Assessment Record for Primary Learning Target, etc.); list the lesson objective(s) assessed on each daily assessment record).
Baseline Data (submit 1 copy of the completed baseline data for both learning targets in a single file, labeling them to identify them as the assessment for the primary vs. supporting/secondary learning target. You may include the baseline data at the beginning of the daily assessment records for each learning target or submit as a separate file).
Part C. Evidence of Feedback (document the location of your evidence of feedback in the Assessment Commentary; if feedback is not written on the student work samples or recorded on the video clips, submit only 1 file for each student, document, video file, OR audio file; if you submit feedback as a video or audio clip and comments cannot be clearly heard, attach transcriptions of your comments (no more than 2 pages) to the end of the Assessment Commentary).
For Use of Communication-If you choose to submit a video clip of learner use of communication that is not included in Task 2, it should be no more than 5 minutes, with a time stamp reference for the evidence of language used described in the Assessment Commentary.
Part D. Assessment Commentary (no more than 8 pages of commentary, including prompts; no more than 5 pages for the chosen assessment; if necessary no more than 2 pages of feedback transcriptions; use Arial 11-point type; single space with 1” margins on all sides; insert documentation at the end of the commentary file if portions of the work sample, feedback, or evidence of language use are in Braille or in a language other than English OR if communication in sign between the candidate and focus learner cannot be seen.
Important: attach a blank copy of the work sample assessment, including directions/prompts provided to the focus learner.
