JOB DESCRIPTION

POST:Registered Nurse – Band 5

RESPONSIBLE TO:Ward sister

RESPONSIBLE FOR:Effective planning, co-ordinating and delivery of patient care.

MAIN PURPOSE OF THE ROLE:

To provide specialist palliative care to patients, mentor students and newly qualified staff and participate in ward management under the direction of the Ward sister.

MAIN DUTIES AND RESPONSIBILITIES:

Under the direction of the Ward sister and in collaboration with the multi-disciplinary team the post holder will contribute to the effective running and development of the Incare area and support other the other hospice care areas by:

  • Carrying out holistic nursing assessments of patients and their family/carers working in partnership with the patient and carers.
  • Formulate a plan of care with the patients and their families to satisfy their nursing care needs.
  • Continually re-assess and evaluate the effectiveness of care delivered to the patient and support to carers.
  • Plan and organise the discharge of patients in accordance with the Hospice Discharge Policy and in liaison with the Discharge Co-ordinator.
  • Maintain clear, accurate, legible and up-to-date nursing documentation that provides an accurate record of nursing care delivered and ensure data is entered on palcare.
  • Deliver direct patient care to patients and their families/carers.
  • Contribute to the development of nursing practice using an evidence-based approach to care delivery.
  • Undertake additional developmental responsibilities as negotiated and allocated by the Ward sister or Clinical Services Manager.
  • Support the Ward sister in providing statistical information to the Clinical Services Manager.
  • Provide leadership and supervision for, and empower junior nurses/health care assistants and volunteer ward based staff.
  • To act as co-ordinator of the ward in support of, or in the absence of the Ward sister.
  • Maintain effective communication (both written and verbal) within the nursing team and the multi professional team and with outside agencies as appropriate.
  • Be aware of financial constraints when prescribing and delivering nursing care.
  • Supportthe Ward sisterin managing change and development within the hospice.
  • Actively pursue your own personal and professional development as identified at appraisal and utilise those personal skills to enhance the overall quality of care within the Hospice
  • Use knowledge and skills to educate peers in a variety of ways on an ongoing basis.
  • Act as a mentor/preceptor to junior nursing staff and students.
  • Contribute to an effective learning environment within the Hospice.
  • Participate in audit and research with the Hospice environment and share knowledge gained with the team.
