TEACH-OUT APPROVAL FORM

Chapter III.A.7. of the Accreditation Manual outlines when an institution or program is required to submit a teach-out plan. A proposed Teach-out plan must be accompanied by the Teach-out Approval Form. Teach-out materials may be found under the Forms Tab on the ABHES Website at All materials must be submitted on a USB (stick) drive or on a CD Rom and labeled as a teach-out plan with the (1) institution’s name, (2) city/state, (3) ABHES ID #, and (4) the submission date. Materials should be sent to the attention of Amanda Ludwa.

The institution/program has been directed by the Commission to submit a proposed teach-out plan.

Proposed Teach-out plan is a precautionary measure directed by the Commission in an effort to safe-guard current students and obtain demographic information on the program(s) offered by an institution. This does not represent an executed teach-out until directed otherwise by the Commission.

The institution/program is voluntarily teaching-out its program(s).

The institution is closing (please complete and submit with this form a Closing of Campus form).

The institution/program is conducting a self teach-out.

The institution/program is conducting a teach-out with another institution(s).

Please note below the institution(s) that would be considered for a proposed Teach-out.

Please note: Contact or agreement with the proposed teach-out institution is NOT required for the Teach-out Plan. Such contact IS required when a Teach-out Agreement with a separate institution is submitted.

The teach-out plan must be consistent with applicable standards and regulations and provide for the equitable treatment of students. Teach-out materials must include, but are not limited to, exhibits to evidence that:

a)The teach-out institution is accredited by an institutional accrediting agency recognized by the United States Department of Education.

b)The teach-out institution is stable, carries out its mission and meets all obligations to existing students.

c)The teach-out institution has the necessary experience, resources and support services to provide educational program(s) that are of acceptable quality and reasonably similar in content, structure and scheduling to those provided by the institution or program that may cease operations either entirely or at one of its locations. This includes graduates’ access to the same credentialing exams, as applicable.

d)Students are provided access to the program and services without requiring them to move or travel substantial distances.

e)Students will be treated equitably with regard to any charges or refunds; and, if tuition has been paid in full, that students receive the instruction promised at no additional charge.

f)Students will be provided with written notification regarding their rights and opportunities for teach-out, transfer and refunds.

In addition to exhibits evidencing the equitable treatment of students as outlined above, all institutions or programs must submit the following:

  1. An Excel spreadsheet listing all current students to include: a contact telephone number; a mailing address; email addresses; their standing in their program(s) of study (credits earned and estimated completion date); and, any financial responsibilities (unearned tuition, refunds due or additional charges pending if tuition is not paid in full.)
  1. A Sample written notification to students regarding their rights and opportunities for teach-out, transfer and refunds; and, the proposed delivery method of notification.
  1. The location where student records, including academic and financial records, would be maintained, should the institution close. The institution or program must include a contact phone number as address.
